揿针联合血府逐瘀汤治疗缺血性心肌病心功能不全临床研究

摘　　要：目的:观察揿针联合血府逐瘀汤治疗缺血性心肌病心功能不全的临床疗效。方法:选取2023年7月至2024年8月河南中医药大学第一附属医院收治的124例缺血性心肌病心功能不全患者为研究对象,按照随机数字表法分为研究组和对照组,每组各62例。对照组给予常规治疗,研究组在对照组治疗的基础上给予揿针联合血府逐瘀汤治疗。比较两组患者的临床疗效及治疗前后中医证候积分、心功能指标[左心室射血分数(left ventricular ejection fraction,LVEF)、左心室收缩末期内径(left ventricular end-systolic dimension,LVESD)、左心室舒张末期内径(left ventricular end-diastolic diameter,LVEDD)、每搏输出量(stroke volume,SV)、左心室质量指数(left ventricular mass index,LVMI)]、生化指标[N端脑钠肽前体(N-terminal pro-B type natriuretic peptide,NT-proBNP)]和心力衰竭程度变化情况。结果:研究组有效率为91.94%,显著高于对照组的77.42%(P<0.05)。两组患者治疗后中医证候积分低于本组治疗前,治疗后研究组心痛、心悸、胸闷、气短和乏力等症状评分分别为(2.03±0.21)分、(1.26±0.28)分、(1.35±0.31)分、(1.52±0.26)分和(1.42±0.28)分,低于对照组(P<0.05)。两组患者治疗后LVEF、SV水平高于本组治疗前,LVESD、LVEDD和LVMI水平低于本组治疗前,且治疗后组间比较,差异具有统计学意义(P<0.05)。两组患者治疗后NT-proBNP水平低于本组治疗前,且治疗后研究组NT-proBNP水平为(592.27±142.13)ng·L-1,低于对照组,差异有统计学意义(P<0.05)。两组患者治疗后各项心力衰竭症状评分低于本组治疗前,且治疗后研究组低于对照组(P<0.05)。结论:揿针联合血府逐瘀汤治疗缺血性心肌病心功能不全疗效确切,可改善患者的临床症状、心功能指标、生化指标及心力衰竭程度。Objective:To evaluate the clinical efficacy of thumbtack needle combined with Blood Mansion Stasis-Expelling Decoction on cardiac dysfunction in ischemic cardiomyopathy.Methods:A total of 124 patients with cardiac dysfunction in ischemic cardiomyopathy admitted to First Affiliated Hospital of Henan University of Chinese Medicine from July 2023 to August 2024 were randomized into the study group and the control group using the random number table,with 62 cases in each group.The control group received conventional therapy,while the study group received additional thumbtack needle therapy combined with Blood Mansion Stasis-Expelling Decoction.Clinical efficacy,and the changes in TCM syndrome scores,cardiac function,biochemical indicators and heart failure severity before and after treatment were compared between the two groups.Results:The study group exhibited a significantly higher total efficacy rate(P<0.05).After treatment,TCM syndrome scores in both groups decreased than those before treatment,Post-treatment for chest pain(2.03±0.21),palpitations(1.26±0.28),chest tightness(1.35±0.31),shortness of breath(1.52±0.26),and fatigue(1.42±0.28)were significantly lower than those of the control group(P<0.05).After treatment,the levels of LVEF and SV in both groups were higher than those before treatment,while the levels of LVESD,LVEDD,and LVMI were significantly lower than those before treatment.The differences between the groups were statistically significant(P<0.05).The NT-proBNP level both groups were lower than that before treatment,and the NT-proBNP level after treatment was(592.27±142.13)ng·L-1,which was lower than that of the control group(P<0.05).The scores of various heart failure symptoms of both groups after treatment were lower than those before treatment,and the scores of the study group were lower than those of the control group(P<0.05).Conclusion:Thumbtack needle combined with Blood Mansion Stasis-Expelling Decoction demonstrates definitive efficacy on patients with cardiac dysfunction in ischemic c
