摘　　要：艾灸治疗膝骨关节炎(knee osteoarthritis,KOA)的作用机制主要有:艾灸调节炎症细胞因子发挥抗炎作用;艾灸对膝关节疼痛的缓解机制(包括神经调节和痛觉传导的抑制);艾灸对膝关节功能的恢复机制(包括软组织修复、关节液的调节)等。艾灸治疗KOA,仍需大规模、严格设计的临床试验和机制研究,以全面验证其疗效;建立统一的艾灸治疗标准和操作规范,以提高研究结果的可重复性和可比性;深入探讨艾灸在分子和细胞水平上的具体作用机制,揭示其抗炎、镇痛和功能恢复的生物学基础;开展大规模、多中心的随机对照试验,验证艾灸治疗KOA的临床疗效和长期安全性;综合运用影像学、生物标志物等多种评价指标,全面评估艾灸的治疗效果。The therapeutic mechanisms of moxibustion for knee osteoarthritis(KOA)primarily include:Anti-inflammatory effects through regulation of inflammatory cytokines;the mechanism of moxibustion in relieving knee joint pain(including neural regulation and inhibition of pain conduction);functional restoration mechanisms encompassing soft tissue repair and synovial fluid regulation.To advance moxibustion therapy for KOA,the following research priorities should be emphasized:conducting large-scale,rigorously designed clinical trials and mechanistic studies to comprehensively validate therapeutic efficacy;establishing unified treatment protocols and operational standards to enhance reproducibility and comparability of research outcomes;elucidating molecular and cellular mechanisms underlying its anti-inflammatory,analgesic,and regenerative effects;implementing multicenter randomized controlled trials to verify long-term clinical efficacy and safety;and adopting multimodal evaluation systems incorporating imaging and biomarkers for comprehensive therapeutic evaluation.
