摘 要:目的:探讨慢性病对老年人主观幸福感的影响及其作用机制。方法:采用2018年和2020年中国健康与养老追踪调查数据,运用工具变量法和条件混合过程估计法实证检验异质性的相关因素。结果:慢性病对老年人主观幸福感具有显著负向影响,老年人患慢性病的数量越多,其主观幸福感水平越低。个体异质性分析表明,慢性病对老年人主观幸福感的负向影响在男性、低龄以及乡村地区群体中表现更为明显。病种异质性分析表明,对于恶性肿瘤患者、胃部疾病、肝脏疾病患者、肾脏疾病患者和情感及精神问题患者来说,慢性病多病共存对其主观幸福感的影响不显著。结论:慢性病主要通过降低身体健康水平和心理健康水平、提高门诊医疗负担以及减少农业劳动供给时间三条途径,降低老年人主观幸福感。Objective:To explore the impact of chronic diseases on the subjective well-being of older adults and its mechanism of action.Methods:Data from the 2018 and 2020 China Health and Retirement Longitudinal Survey(CHARLS)were used to empirically test the correlates of heterogeneity using the instrumental variable method and conditional mixed process estimation.Results:Chronic diseases have a significant negative effect on the subjective well-being of older adults,and the higher the number of chronic diseases older adults suffer from,the lower their level of subjective well-being.Individual heterogeneity analyses showed that the above negative effects were more significant in male,younger,and rural elderly groups.Disease heterogeneity analysis showed that the effect of chronic disease multimorbidity on subjective well-being was not significant for patients with malignant tumors,gastric diseases,liver diseases,kidney diseases,and emotional and mental problems.Conclusion:Chronic diseases reduce the subjective well-being of the elderly mainly through three pathways:lowering the level of physical health and mental health,increasing the burden of outpatient medical care,and reducing the supply time of agricultural labor.
