摘  要:目的探讨Toll样受体(TLR)3基因DNA甲基化与乙型肝炎病毒(HBV)母婴阻断效果的关联性。方法选取2021年9月至2022年12月广西柳州市34家医院(三级甲等妇幼保健院1家、二级甲等妇幼保健院5家、一级乡镇卫生院28家)的乙型肝炎表面抗原(HBsAg)阳性母亲所产婴幼儿1205例作为研究对象,评价婴幼儿的HBV母婴阻断效果。选择HBsAg阴性婴幼儿作为对照组,HBsAg阳性婴幼儿作为病例组。收集两组外周静脉血标本提取DNA,对TLR3基因进行甲基化检测。结果有9例婴幼儿为HBsAg阳性,HBV母婴阻断失败率为0.75%;1196例婴幼儿为HBsAg阴性,HBV阻断成功率为99.25%。病例组cg17081048、cg25367691位点DNA甲基化水平均高于对照组,cg15851014、cg01481910、cg19849992、cg01036205、cg00305797位点DNA甲基化水平均低于对照组(P<0.05)。cg17081048、cg01481910、cg19849992位点处于基因间区域;cg25367691位点处于基因的第1个外显子区域;cg15851014、cg01036205、cg00305797位点处于转录起始位点上游200~1500 bp。结论广西柳州市乙型肝炎母婴传播综合干预效果较好。TLR3基因cg17081048、cg25367691、cg15851014、cg01481910、cg19849992、cg01036205、cg00305797位点的DNA甲基化水平可能与HBV母婴阻断效果的发生有关。Objective To explore relationship between DNA methylation of Toll-like receptor 3(TLR)gene and blocking effect of hepatitis B virus(HBV)mother-to-child transmission.Methods A total of 1205 infants and young children born to hepatitis B surface antigen(HBsAg)positive mothers from 34 hospitals(one tertiary grade A maternal and child health hospital,five secondary grade A maternal and child health hospitals,28 primary township health centers)in Liuzhou City,Guangxi Zhuang Autonomous Region from September 2021 to December 2022 were selected as research subjects.Blocking effect of HBV mother-to-child transmission in infants and young children was evaluated.HBsAg negative infants and young children were selected as the control group,HBsAg positive infants and young children were selected as the case group.Peripheral venous blood samples of the two groups were collected,DNA was extracted,methylation of TLR3 gene was detected.Results Nine infants and young children were HBsAg positive,blocking failure rate of HBV mother-to-child transmission was 0.75%;1196 infants and young children were HBsAg negative,blocking success rate of HBV mother-to-child transmission was 99.25%.DNA methylation levels at positions cg17081048 and cg25367691 in the case group were higher than those in the control group,DNA methylation levels at positions cg15851014,cg01481910,cg19849992,cg01036205,and cg00305797 in the case group were lower than those in the control group(P<0.05).cg17081048,cg01481910,and cg19849992 positions were located in the intergenic region;cg25367691 position was located in the first exon region of the gene;cg15851014,cg01036205,and cg00305797 positions were located 200-1500 bp upstream of the transcription start site.Conclusion The comprehensive intervention effect of mother-to-child transmission of HBV in Liuzhou City,Guangxi Zhuang Autonomous Region is relatively good.DNA methylation levels at positions cg17081048,cg25367691,cg15851014,cg01481910,cg19849992,cg01036205,and cg00305797 of TLR3 gene may be associated with the
