基于中医阴阳理论探讨肠道菌群与慢传输型便秘的相关性研究进展

摘　　要：慢传输型便秘(STC)以结肠传输时间延长为特征,是常见的胃肠道疾病之一。肠道菌群作为影响肠道功能的重要因素,与STC的发病密切相关。肠道菌群及其通过胆汁酸与5-羟色胺信号系统发挥的胃肠动力调节作用在肠道稳态中表现出平衡倾向、相互拮抗、相互促进等特点,与中医阴阳理论中阴阳的消长平衡、对立制约、互根互用等基本规律相似。本文从中医阴阳观出发探讨肠道菌群与STC的关系,综述肠道菌群的结构与功能、5-羟色胺和胆汁酸水平变化对肠道动力的调节及阴阳相应之法。在中医阴阳理论指导下,以中医药治疗恢复肠道菌群的平衡状态,可作为临床治疗STC的新思路。Slow transit constipation(STC)is characterized by prolonged colonic transit time,is one of the common gastrointestinal diseases.Intestinal flora as an important factor affecting intestinal function,is closely related to the onset of STC.Intestinal flora and its gastrointestinal motility regulation through bile acid and 5-hydroxytryptamine signaling system exhibit characteristics of balance,mutual antagonism,and mutual promotion in intestinal homeostasis,similar to basic laws of yin-yang balance,opposition constraint,and mutual root interaction in yin-yang theory of traditional Chinese medicine.This article explores correlation between intestinal flora and STC from the perspective of yin-yang theory of traditional Chinese medicine,summarizes structure and function of intestinal flora,the regulation of gut motility by changes in 5-hydroxytryptamine and bile acid levels,and corresponding method of yin-yang.Guided by yin-yang theory of traditional Chinese medicine,the use of traditional Chinese medicine and pharmacy to restore balance of intestinal flora,can serve as a new approach for clinical treatment of STC.
