基于“督脉为阳脉之海”理论探讨调脊通督针刀松解术治疗神经根型颈椎病

摘　　要：神经根型颈椎病(CSR)临床发病率越来越高,症状表现明显,影响正常工作生活。常用的西药及手术治疗大多产生一定的副作用,且效果不一。丁德光主任医师基于“督为阳脉之海”理论,从筋结、阳损两个方面分析CSR的病因病机,提出主要选用督脉点、夹脊穴及棘突旁阳性点的调脊通督针刀松解术,从调脊、通督、复阳3个方面针对性治疗,以改善临床症状、恢复正常功能。本文对调脊通督针刀松解术展开介绍,探讨该法的治疗原理,提出临床标准化操作方法,指导如何选取针刀部位、运用针刀手法,为临床治疗CSR提供思路与方法。Clinical incidence rate of cervical spondylotic radiculopathy(CSR)is getting higher and higher,symptoms are obvious,affecting normal work and life.Commonly used western medicine and surgical treatments often have certain side effects,and the effects vary.Chief Physician Ding Deguang,based on the theory of“governor channel is sea of yang channel”,analyzes etiology and pathogenesis of CSR from two aspects:muscle structure and yang loss,proposes the use of acupotomy lysis with tiaoji tongdu mainly to select governor channel point,jioin acupoint,and positive points near spinous process,targeted treatment is carried out from three aspects:tiaoji,tongdu,fuyang to improve clinical symptoms,and restore normal function.This article introduces technique of acupotomy lysis with tiaoji tongdu,explores treatment principle of this method,proposes standardized clinical operating methods to guide how to select acupuncture knife site and apply acupuncture knife techniques,so as to provide ideas and methods for clinical treatment of CSR.
