改良鞘管和宫腔球囊造影管行子宫输卵管造影的比较研究

摘　　要：目的比较宫腔球囊造影管和改良鞘管行子宫输卵管造影检查的差异。方法回顾性分析2023年1月—12月行子宫输卵管造影(HSG)检查的不孕女性患者图像。采用秩和检验比较鞘管和球囊造影管HSG的时间差异是否有统计学意义。采用卡方检验比较鞘管和球囊造影管HSG术后输卵管显影率、对比剂内渗发生率差异是否有统计学意义。采用t检验比较鞘管HSG有顺产史患者和无顺产史患者对比剂使用量差异是否有统计学意义。结果鞘管HSG患者437例,显影率93.4%,使用宫腔球囊管患者373例,显影率为83.4%。造影时间以中位数和四分位间距表示,分别为312(308,323)s、423(407,456)s。碘海醇内渗发生率分别为1.8%、7.5%。两种造影方法在操作时间、输卵管显影率及不良反应发生率方面的差异有统计学意义(P<0.001)。鞘管HSG有顺产史患者和无顺产史患者对比剂使用量差异无统计学意义(P>0.05)。结论改良鞘管HSG操作简单,省时,患者痛苦小,且造影过程中对比剂内渗发生率低,可替代宫腔球囊管。Objective To compare the effectiveness of intrauterine modified sheath catheter and balloon catheter hysterosalpingography(HSG).Methods The HSG images of infertile women from January to December 2023 were retrospectively analyzed.The rank sum test was used to compare the duration of the HSG and Chi-square test was performed to compare the fallopian tube visualization rates and incidence of contrast intravasation between the two catheters.Student t-test was used to compare the amount of contrast agent required in modified sheath HSG between patients with and without history of vaginal delivery.Results The fallopian tube visualization rate was 93.4%among the 437 patients who underwent modified sheath HSG was significantly(P<0.001)greater than that of balloon catheter HSG(83.4%,373 patients).The duration of HSG(median:312 s,interquartile range:308-323 s)and incidence of contrast intravasation(1.8%)in modified sheath HSG were significantly(P<0.001)lower than that of balloon catheter HSG(423 s,407-456 s,7.5%).There was no significant difference in the amount of contrast required for modified sheath HSG between patients with and without history of vaginal delivery(P>0.05).Conclusion The modified sheath HSG is simple,time-saving,less painful,has a low incidence of intravasation,and can replace the uterine balloon catheter.
