摘　　要：目的探究柴黄颗粒联合孟鲁司特钠治疗小儿慢性咳嗽的效果及对血清微小核糖核酸(miR)⁃138、白介素(IL)⁃17、IL⁃33的影响。方法纳入2023年1月至2024年4月期间太原钢铁(集团)有限公司总医院收治的116例慢性咳嗽患儿,以治疗方案不同分为观察组(n=60)与对照组(n=56)。对照组予以孟鲁司特钠片治疗,观察组基于对照组联合使用柴黄颗粒治疗。比较两组疗效、症状改善时间(气促、咳嗽及肺啰音消失时间)、药物不良反应,治疗前后免疫功能[CD_(3)^(+)、CD_(4)^(+)、CD_(8)^(+)]以及血清miR⁃138、IL⁃17、IL⁃33水平。结果观察组总有效率为86.87%,较对照组的62.50%,显著更高,差异具有统计学意义(P<0.05);观察组的气促、咳嗽及肺啰音消失时间均明显短于对照组,差异具有统计学意义(P<0.05);两组治疗后的CD_(3)^(+)、CD_(4)^(+)较治疗前均升高,而CD_(8)^(+)则均降低,且观察组改善更优,差异具有统计学意义(P<0.05);两组治疗后的miR⁃138、IL⁃17、IL⁃33较治疗前均降低,且观察组显著更低,差异具有统计学意义(P<0.05);两组治疗期间的药物不良反应比较,差异无统计学意义(P>0.05)。结论柴黄颗粒联合孟鲁司特钠可显著提高小儿慢性咳嗽疗效,减轻气道炎症,缓解咳嗽症状,其可能与抑制机体内miR⁃138、IL⁃17及IL⁃33表达有关。Objective To explore the effect of Chaihuang granule combined with montelukast sodium in the treatment of children with chronic cough and its effect on serum miR⁃138,IL⁃17,and IL⁃33.Methods In this study,116 children with chronic cough were included from January 2023 to April 2024 at General Hospital of Taiyuan Iron and Steel(Group)Co.,Ltd.They were divided into two groups based on different treatment plans:the observation group(n=60)and the control group(n=56).The control group was treated with Montelukast sodium tablets,and the observation group received Chaihuang granules in addition to Montelukast sodium tablets.The efficacy,symptom improvement time(time for dyspnea,cough,and lung rales to disappear),adverse drug reactions,immune function(CD_(3)^(+),CD_(4)^(+),CD_(8)^(+)),and serum miR⁃138,IL⁃17,and IL⁃33 levels before and after treatment were compared between the two groups.Results The overall effective rate of the observation group was 86.87%,which was significantly higher than the 62.50%seen in the control group(P<0.05).The time for cough,dyspnea,and lung rales to vanish in the observation group was shorter than that in the control group,showing statistically significant differences(P<0.05).After treatment,CD_(3)^(+)and CD_(4)^(+)levels in both groups were higher than before treatment,while CD_(8)^(+)was lower,and the improvement was better in the observation group,the difference was statistically significant(P<0.05).After treatment,the levels of miR⁃138,IL⁃17,and IL⁃33 in both groups were lower than those before treatment,and the levels in the observation group were significantly lower(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups during the treatment period(P<0.05).Conclusion Chaihuang granules combined with montelukast sodium can significantly improve the efficacy of chronic cough in children.This combination can reduce airway inflammation and relieve cough symptoms.Thses positive effects may be related to the inh
