摘　　要：目的分析急性心肌炎患者预后危重症化进展为暴发性心肌炎的影响因素。方法回顾性收集2016年1月~2024年8月临沂市人民医院收治的急性心肌炎患者资料,共187例,其中暴发性心肌炎48例(FM组),非暴发性心肌炎139例(非FM组)。比较FM组与非FM组在症状、生命体征、心电图参数、超声心动图指标及实验室检查指标方面的差异,并对有显著差异的因素进行logistic多因素回归分析。结果logistic回归分析显示,心电图QRS间期延长、NT-proBNP水平升高、收缩压降低及LVEF降低为暴发性心肌炎的独立风险因素(P<0.05)。结论心电图QRS间期延长、NT-proBNP水平升高、收缩压降低及LVEF降低是急性心肌炎进展为暴发性心肌炎的独立预后因素。应加强对上述高风险患者的动态监测,进行早期识别与干预。Objective To analyze the influencing factors for the progression of acute myocarditis to fulminant myocarditis.Methods Retrospective data were collected from patients with acute myocarditis admitted between January 2016 and August 2024.A total of 187 patients were included,including 48 cases of fulminant myocarditis(FM group)and 139 cases of non-fulminant myocarditis(non-FM group).Symptoms,vital signs,electrocardiogram parameters,echocardiographic indicators,and laboratory test results were compared between the FM group and the non-FM group.Logistic multivariate regression analysis was performed on factors with significant differences.Results Logistic regression analysis showed that prolonged QRS interval,elevated NT-proBNP levels,decreased systolic blood pressure,and reduced LVEF were independent risk factors for the development of fulminant myocarditis.Conclusion Prolonged QRS interval,elevated NT-proBNP levels,decreased systolic blood pressure,and reduced LVEF are independent risk factors for fulminant myocarditis in patients with acute myocarditis.Dynamic monitoring and early identification and intervention for patients with these risks should be emphasized.
