摘 要:目的探讨盐酸戊乙奎醚注射液(PHC)在有机磷中毒(OPP)中的应用效果。方法前瞻性选取2021年1月~2023年12月利辛县人民医院收治的156例OPP患者,随机数表法分为对照组和观察组各78例。对照组静脉注射阿托品,观察组肌内注射PHC,比较两组患者疗效、临床恢复时间、谵妄开始(谵妄发作时间)和持续时间等指标。结果观察组总有效率高于对照组(93.59%vs.78.21%),差异有统计学意义(χ^(2)=7.620,P=0.006)。观察组患者轻、中、重度的谵妄开始时间[(6.48±2.45)h、(9.48±1.36)h、(13.67±2.32)h]长于对照组,差异有统计学意义(t=8.809、8.811、15.954,P<0.05);观察组患者轻、中、重度的谵妄持续时间[(17.64±3.49)h、(13.69±3.26)h、(8.29±2.13)h]均长于对照组,差异有统计学意义(t=17.192、12.756、14.670,P<0.05)。观察组乳酸脱氢酶(LDH)(116.79±22.34)、肌酸激酶同工酶-MB(CK-MB)(16.24±4.30)U∕L、肌酸激酶(CK)(102.47±27.49)U∕L和羟丁酸脱氢酶(HBDH)(152.60±12.34)U∕L均低于对照组,差异有统计学意义(t=5.785、4.990、3.748、7.615,P<0.05)。观察组并发症发生率为6.41%均低于对照组的20.51%,差异有统计学意义(χ^(2)=6.658,P=0.010)。结论PHC在OPP急救中展现独特优势,可实现精准控制中枢症状同时减轻外周不良反应,延缓谵妄发生,减少阿托品中毒风险,使住院、呼吸机使用及CHE恢复时间缩短,总并发症减少,心脏保护作用及整体治疗效率提升,对OPP治疗具有重要价值。Objective The application of penehyquidine hydrochloride injection(PHC)in the treatment of organophosphorus poisoning(OPP).Methods A total of 156 patients with OPP admitted to our hospital from January 2021 to December 2023 were enrolled and evenly divided into a control group and an observation group(78 each)through random draw.The control group received intravenous atropine,while the observation group was administered PHC via intramuscular injection.Clinical outcomes,recovery times,onset and duration of delirium,myocardial enzyme spectrum indices,and complications were compared between the two groups.Results The total effective rate in the observation group was 93.59%higher than that in the control group(χ^(2)=7.620,P=0.006).The onset of mild,moderate and severe delirium in the observation group[(6.48±2.45)h,(9.48±1.36)h,(13.67±2.32)h]was longer than that in the control group(t=8.809,8.811,15.954,P<0.05).The duration of mild,moderate and severe symptoms in the observation group[(17.64±3.49)h,(13.69±3.26)h,(8.29±2.13)h]was longer than that in the control group(t=17.192,12.756,14.670,P<0.05).Lactate dehydrogenase(LDH)(116.79±22.34),creatine kinase isozyme-MB(CX-MB)(16.24±4.30)U∕L,creatine kinase(CK)(102.47±27.49)U∕L and hydroxybutyrate dehydrogenase(HBDH)(152.60±12.34)U∕L were all lower in the observation group According to the group(t=5.785,4.990,3.748,7.615,P<0.05).The complication rate of 6.41%in the observation group was lower than 20.51%in the control group(χ^(2)=6.658,P=0.010).Conclusion PHC shows unique advantages in OPP first aid,achieving accurate control of central symptoms,alleviating peripheral side effects,delaying the occurrence of delirium,reducing the risk of atropine poisoning,shortening hospitalization,ventilator use and CHE recovery time,reducing total complications,cardiac protection and improving overall treatment efficiency,which is of great value for OPP treatment.
