摘  要:目的分析Polo样激酶1(PLK1)、肌球蛋白重链9(MYH9)、第二个线粒体衍生的半胱天冬酶激活蛋白(Smac)在瘢痕疙瘩组织中表达情况及相关性。方法回顾性选取2022年10月~2023年10月于沧州市人民医院进行瘢痕疙瘩组织切除术的97例患者,收集瘢痕疙瘩患者手术切除的瘢痕疙瘩组织和瘢痕疙瘩旁正常皮肤组织标本,在切片后做好免疫组织化学标记,并利用Western Blot法检测PLK1、MYH9的表达,利用PT-PCR检测Smac的表达,分析PLK1、MYH9和Smac在瘢痕疙瘩患者组织中的表达情况,采用多因素logistic回归模型分析瘢痕疙瘩发生的危险因素,分析PLK1、MYH9、Smac与瘢痕疙瘩患者临床特征的相关性,利用受试者工作特征(ROC)曲线分析三者联合检测对瘢痕疙瘩患者预后的预测价值。结果正常皮肤组织中PLK1、MYH9的表达水平分别为(1.13±0.15)(0.95±0.14),低于瘢痕疙瘩组织中的(1.49±0.30)、(1.43±0.20);正常皮肤组织中Smac的表达水平为(1.16±0.13),高于瘢痕疙瘩组织中的(0.62±0.09),差异均有统计学意义(t=10.570、19.364、33.926,P<0.05)。不同家族史、民族、病灶分布位置、病灶数量、病灶最大直径的瘢痕疙瘩患者组织中PLK1、MYH9、Smac的表达水平比较,差异均有统计学意义(P<0.05)。多因素分析结果显示,PLK1、MYH9、Smac表达水平均是瘢痕疙瘩发生的独立危险因素(P<0.05)。Spearman相关性分析结果显示,PLK1、MYH9、Smac的表达水平与瘢痕疙瘩患者的家族史、民族、病灶分布位置、病灶数量和病灶最大直径均呈正相关(P<0.05)。PLK1、MYH9、Smac联合诊断对瘢痕疙瘩患者预后的预测效能最高,灵敏度、特异度、准确度均高于单项指标。结论PLK1、MYH9在瘢痕疙瘩组织中的表达水平升高,Smac在瘢痕疙瘩组织中的表达水平降低,并且与患者临床特征关系密切,可以较好地对瘢痕疙瘩患者的预后进行预测。Objective To investigate the expression patterns and correlations of PLK1,MYH9,and Smac in keloid tissues.Methods Retrospectively,97 patients who underwent keloid tissue resection in our hospital from October 2022 to October 2023 were selected as the research objects of this study.Samples of keloid tissue and paracoid normal tissue surgically removed from keloid patients were collected,and immunohistochemical markers were made after section.Furthermore,Western Blot was used to detect the expressions of PLK1 and MYH9,PT-PCR was used to detect the expression of Smac,and the expressions of PLK1,MYH9 and Smac in clinical symptoms of keloid patients were analyzed,and the risk factors of keloid occurrence were analyzed by multi-factor Logistic.Spearman correlation analysis was conducted to analyze the correlation between PLK1,MYH9,Smac and clinical features of patients,and ROC curve was used to analyze the predictive value of the combined detection of PLK1,MYH9,and SMAC in patients with keloid.Results In normal skin tissue,the expression levels of PLK1(1.13±0.15)and MYH9(0.95±0.14)were found to be lower than those in keloid tissue,where PLK1 was 1.49±0.30 and MYH9 was 1.43±0.20.The expression level of Smac(1.16±0.13)in normal skin tissue was higher than that in keloid tissue(0.62±0.09),and the difference was statistically significant(t=10.570,19.364,33.926,P<0.05).There were significant differences in the expressions of PLK1,MYH9 and Smac in family history,nationality,location of lesions,number of lesions and maximum diameter of lesions in patients with keloid(P<0.05).Logistic regression analysis showed that the expressions of PLK1,MYH9 and Smac were risk factors for keloid development(P<0.05).Spearman correlation analysis showed that PLK1,MYH9 and Smac were positively correlated with family history,nationality,location of lesions,number of lesions and maximum diameter of lesions(P<0.05).ROC curve analysis showed that the single diagnosis of PLK1,MYH9 and Smac was significantly lower than the combined diagnosis o
