摘　　要：原发性肾病综合征(PNS)是一种复杂的疾病,其发病率呈逐年上升趋势。该病由多种原因导致肾小球基膜通透性增加而引起。西医治疗通常采用激素和免疫抑制剂,虽然可缓解病情,但存在药物依赖性强、不良反应多、远期效果不佳等问题。中医学认为PNS与肺脾肾脏腑虚弱、阳气虚衰等因素有关,治疗时需辨证论治,强化脏腑对水液的调节能力。中医治疗在消除临床症状、减轻西药不良反应、改善预后等方面具有一定的优势。现代药理研究显示,中药可通过抑制免疫炎症反应、调节蛋白通路、调节水和电解质平衡、抑制足细胞凋亡等多种途径减轻肾脏损伤,治疗PNS具有多靶点、多途径的优势,且安全有效。本文对PNS的中西医发病机制及中医药的应用现状、作用机制进行综述,为开展相关研究提供参考。Primary nephrotic syndrome(PNS)is a complex disease with a gradual increase in incidence rate.It is caused by increased glomerular basal membrane permeability due to a variety of causes.Western medicine treatment usually uses hormones and immunosuppressants,which can alleviate the condition,but there are problems such as strong drug dependence,multiple adverse reaction,and poor long-term effect.Traditional Chinese medicine believes that PNS is related to factors such as weakness of lung,spleen,kidney,as well as deficiency of yang qi.Treatment should be based on syndrome differentiation,strengthen the regulation ability of zang fu viscera on fluids.Traditional Chinese medicine treatment has certain advantages in eliminating clinical symptoms,reducing adverse reactions to western medicine,and improving prognosis.Modern pharmacological research has shown that traditional Chinese medicine can alleviate kidney damage through various pathways such as inhibiting immune inflammatory reaction,regulating protein pathway,regulating water and electrolyte balance,and inhibiting podocyte apoptosis.It has the advantages of multi-target and multi pathway treatment for PNS,and is safe and effective.This article reviews the pathogenesis of PNS in both traditional and western medicine,as well as current status and action mechanism of traditional Chinese medicine and pharmacy,providing reference for related research.
