抗凝门诊的患者教育对华法林治疗依从性和患者满意度的影响研究

摘  要:目的:调查抗凝门诊药师的患者教育对于接受华法林治疗患者的华法林认知程度、服药依从性和药师服务满意度的影响。方法:针对2023年1月~2024年7月期间曾就诊抗凝门诊接受过药师华法林用药指导的患者,根据患者意愿,采用电子调查问卷的方式收集患者的问卷调研信息和临床信息。结果:最终回收有效问卷84份(有效回收率69.42%)。72.62%的患者基本了解华法林相关知识,17.86%的患者非常熟悉华法林相关知识,其余患者表示还不太了解华法林的相关知识。患者的整体依从性评分为(6.53±1.38)分,其中25.00%(21例)的患者服用华法林依从性高,47.62%(40例)的患者服药依从性中等,27.38%(23例)的患者服用华法林依从性低。华法林治疗依从性与监测INR是否便利、调整华法林剂量的医生、华法林知识掌握程度相关(P<0.05)。患者对抗凝门诊整体满意度和药师服务满意度较高,且相较于不太熟悉华法林知识的患者,非常熟悉华法林知识的患者向周围人推荐抗凝门诊的可能性更高(P<0.05)。结论:药师应发挥专业特长,为患者定制个体化的用药教育、加强随访,从而提高患者对华法林的认知度、减轻患者对华法林不良反应的焦虑,提高服药依从性,进一步提升抗凝质量,提高患者满意度。Objective:To investigate the effect of patient education provided by pharmacists in the anticoagulation clinic on warfarin knowledge,treatment compliance,and satisfaction with pharmacist services in patients receiving warfarin treatment.Methods:Patients who visited the anticoagulation clinic and received warfarin medication guidance from pharmacists from January 2023 to July 2024,were collected for patient questionnaire data and clinical information through electronic surveys based on their willingness.Results:A total of 84 valid questionnaires were ultimately collected(with a valid response rate of 69.42%).72.62%of patients had a basic understanding of warfarin-related knowledge,and 17.86%were very familiar with warfarin-related knowledge,while the remaining patients reported having limited knowledge about warfarin.The overall compliance score of patients was(6.53±1.38),with 25.00%of patients(21 patients)showing high warfarin treatment compliance,47.62%(40 patients)showing moderate treatment compliance,and 27.38%(23 patients)showing low treatment compliance.Warfarin treatment compliance was associated with the convenience of monitoring INR,the physician who adjusted the warfarin dosage,and the level of knowledge about warfarin(P<0.05).Patients expressed high satisfaction with the anticoagulation clinic and pharmacist services.Additionally,compared to patients with limited knowledge of warfarin,those who were very familiar with warfarin-related knowledge were more likely to recommend the anticoagulation clinic to others(P<0.05).Conclusion:Pharmacists should leverage their professional expertise to provide individualized medication education for patients,strengthen follow-up care,and thereby enhance patients'knowledge of warfarin,alleviate their anxiety regarding adverse reactions,improve treatment compliance,and further enhance anticoagulation quality and patient satisfaction.
