某院基于药物治疗管理的咳喘药学门诊服务对哮喘患者的应用效果评价

摘　　要：目的:建立基于药物治疗管理(MTM)服务模式的咳喘药学门诊,评价应用该管理模式下的咳喘药学门诊服务对哮喘患者的临床应用效果。方法:选取100例在某院接受咳喘药学门诊服务的哮喘患者,采用自身对照研究,观察在接受基于MTM咳喘药学服务模式前、干预后(干预3个月和6个月),患者哮喘控制测试(ACT)评分、吸入装置使用得分率、不良反应发生率、 Morisky服药依从性量表(MMAS-8)评分、健康效用值及患者满意度变化情况。结果:该院形成了基于MTM的咳喘药学门诊服务模式。经3个月干预后患者ACT评分由干预前16.93分上升至20.05分;吸入装置使用得分率由38.82%上升至82.18%;不良反应发生率由28.00%下降至13.00%;健康效用值由0.54上升至0.55;依从性评分由(5.46±1.18)分上升至(6.79±0.76)分;患者满意度评分由(36.94±8.49)分上升至(48.62±5.83)分,均具有统计学差异(P<0.05)。且随着干预时间延长至6个月,ACT评分、吸入装置使用得分率、依从性及满意度评分均较干预3个月升高(P<0.05)。结论:基于MTM模式的咳喘药学门诊服务可以提高哮喘患者使用吸入装置的正确率,提高用药依从性,减少药物不良反应发生,保证药物治疗效果最大化,促进哮喘患者疾病得到有效控制。Objective:To establish a cough and asthma pharmacy clinic based on medication therapy management(MTM)service model,and evaluate the clinical application effect of cough and asthma pharmacy clinic services under this management model for asthma patients.Methods:A total of 100 asthma patients receiving cough and asthma pharmacy clinic services at a hospital were selected for a self-controlled study.The changes in asthma control test(ACT)scores,score rate of inhaler device usage,incidence of adverse reactions,Morisky Medication Adherence Scale(MMAS-8)scores,health utility values,and patient satisfaction were observed before and after the intervention(at 3 months and 6 months of intervention)under the MTM-based cough and asthma pharmacy service model.Results:The hospital has established a cough and asthma pharmacy clinic service model based on MTM.After 3 months of intervention,the ACT score was increased from 16.93 to 20.05;The score rate of inhaler device usage was increased from 38.82%to 82.18%;The incidence of adverse reactions was decreased from 28.00%to 13.00%;The health utility value was increased from 0.54 to 0.55;The adherence score was increased from(5.46±1.18)to(6.79±0.76);And the patient satisfaction score was increased from(36.94±8.49)to(48.62±5.83),all with statistical significance(P<0.05).As the intervention time extended to 6 months,ACT scores,score rate of inhaler device usage,adherence and satisfaction scores were higher than those at 3 months(P<0.05).Conclusion:The MTM-based cough and asthma pharmacy clinic services can improve the rate of correct inhaler device use,enhance medication adherence,reduce the occurrence of adverse drug reactions,maximize therapeutic effects,and promote effective disease control in asthma patients.
