火龙罐联合红桃消肿合剂在预防全膝关节置换术后深静脉血栓形成的应用

摘　　要：目的:观察火龙罐联合红桃消肿合剂在预防全膝关节置换术后深静脉血栓形成(DVT)的应用效果。方法:回顾性分析2023年11月~2024年3月于深圳平乐骨伤科医院老年骨科行全膝关节置换术的66例患者的病历资料,根据患者干预方法分为常规组(常规干预,n=31)和中医组(常规干预的基础上加用火龙罐联合红桃消肿合剂,n=35)。评估两组术后3d、14d下肢肿胀、疼痛情况、中医症状量化评分差异,检测两组凝血纤溶功能指标[凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、凝血酶时间(TT)、血小板(PLT)、血红蛋白(Hb)]水平,统计两组术后DVT发生率。结果:术后14d,两组肿胀程度评分、VAS评分、中医症候积分、PT、APTT、TT水平均显著低于术后3d,且中医组低于常规组,差异具有统计学意义(P<0.05)。术后14d,两组PLT、Hb水平均显著高于术后3d,且中医组高于常规组,差异具有统计学意义(P<0.05)。中医组术后出现DVT发生率(5.71%)低于对照组(29.03%),且差异有统计学意义(P<0.05)。结论:火龙罐联合红桃消肿合剂干预预防全膝关节置换术后深静脉血栓形成的效果显著,可促进患者全膝关节置换术后血液循环,减轻疼痛程度,改善下肢肿胀情况。Objective:To observe the effect of Huolongcan combined with Hongtao Xiaotuo mixture in preventing deep vein thrombosis(DVT)after total knee arthroplasty.Method:Retrospectively analyze the medical records of 66patients who underwent total knee arthroplasty in the Department of Geriatric Orthopaedics,Shenzhen Pingle Orthopaedic Hospital,from November 2023to March 2024,and according to the method of patient intervention,they were divided into the conventional group(conventional intervention,n=31)and the Chinese medicine group(the addition of huolong jar combined with hongtao decongestant composite on the basis of conventional intervention,n=35).We assessed the differences in lower limb swelling,pain,and quantitative scores of TCM symptoms between the two groups in 3d and 14d postoperatively,and tested the levels of coagulation and fibrinolytic function indexes[prothrombin time(PT),activated partial thromboplastin time(APTT),thromboplastin time(TT),platelet(PLT),and hemoglobin(Hb)]in the two groups,and counted the incidence rate of DVT in the two groups postoperatively.Results:14days after surgery,swelling degree score,VAS score,TCM symptom score,PT,APTT and TT levels in 2groups were significantly lower than 3days after surgery,and the TCM group was lower than the conventional group,the difference was statistically significant(P<0.05).On day 14after surgery,PLT and Hb levels in 2groups were significantly higher than those in day 3after surgery,and the TCM group was higher than the conventional group,the difference was statistically significant(P<0.05).The incidence of postoperative DVT in the TCM group(5.71%)was lower than that in the control group(29.03%),and the difference was statistically significant(P<0.05).Conclusion:Huolongtin combined with Hongtao Xiaoedema mixture can significantly prevent deep vein thrombosis after total knee arthroplasty,promote blood circulation after total knee arthroplasty,reduce pain and improve lower limb swelling.
