摘 要:目的探讨TBNK细胞及NKT细胞与复发性自然流产发病的关系。方法选择40例不明原因反复自然流产(URSA)患者、20例健康未孕妇女、38例早孕妇女及16例健康分娩的妇女为研究对象,分别为URSA组、健康未孕组、健康早孕组、健康分娩组,应用免疫荧光标记和流式细胞术检测4组研究对象的外周血TBNK细胞及NKT细胞的表达水平,对比不同组别各类TBNK细胞的绝对计数和百分比,分析URSA患者CD3^(+)CD56^(+)CD16^(+)NKT细胞与Th/Ts、CD19^(+)B细胞的相关性。结果URSA组的CD3^(-)CD16^(+)CD56^(+)NK细胞及CD3^(+)CD16^(+)CD56^(+)NKT细胞的百分比均高于其他3组,而CD3^(+)CD4^(+)百分比则低于其他3组(P<0.05);URSA组的CD3^(-)CD16^(+)CD56^(+)NK细胞绝对计数高于其他3组,但差异无统计学意义(P>0.05);URSA患者CD3^(+)CD56^(+)CD16^(+)NKT细胞与Th/Ts、CD19^(+)B细胞无相关性(P>0.05)。结论TBNK细胞免疫异常及NKT细胞的升高与URSA的发生有一定的关联,临床可通过对TBNK细胞及NKT细胞的分析为URSA患者临床治疗方案的选择提供有益参考。Objective The objective of this study was to examine the correlation between TBNK and NKT cells and recurrent spontaneous abortion.Methods The study included a total of 40 patients with unexplained repeated spontaneous abortion(URSA),20 healthy non-pregnant women,38 women in early pregnancy,and 16 women with normal pregnancy and delivery.These groups were respectively referred to as the URSA group,healthy non-pregnant group,healthy early pregnancy group(8 to 12 weeks),and healthy delivery group.The expression levels of TBNK cells and NKT cells in peripheral blood were assessed using immunofluorescence labeling.Results The URSA group exhibited higher percentages of CD3^(-)CD16^(+)CD56^(+)NK cells and CD3^(+)CD16^(+)CD56^(+)NKT cells compared to the other three groups,with statistical significance(P<0.05).Conversely,the percentages of CD3^(+)CD4^(+)cells were lower in the URSA group compared to the other three groups,also with statistical significance(P<0.05).The absolute count of CD3^(-)CD16^(+)CD56^(+)NK cells in the URSA group was higher than in the other three groups,but this difference did not reach statistical significance(P>0.05).Conclusion The occurrence of URSA is associated with the presence of immune abnormalities in TBNK cells and the increase in NKT cells.
