基于五运六气探讨慢性萎缩性胃炎患者先天运气禀赋特点及治疗策略

摘　　要：【目的】基于五运六气理论探讨慢性萎缩性胃炎患者出生时间运气的分布规律,总结该类患者先天运气禀赋特点,并以此探讨慢性萎缩性胃炎的治疗策略。【方法】选取2018年1月至2023年12月广州中医药大学附属深圳宝安中医院(即深圳市宝安区中医院)收治的2046例慢性萎缩性胃炎患者,运用SPSS 24.0统计软件对其出生时间的五运六气各要素分布情况进行分析,归纳其先天运气禀赋特点,并以此提出相应治疗策略。【结果】出生于岁运为火运不及之年、主运为金运、主气为阳明燥金、客气为太阳寒水、少阳相火司天/厥阴风木在泉的慢性萎缩性胃炎患者比例最高,其中,主运、主气和客气分布的差异有统计学意义(P<0.05或P<0.01),岁运及司天/在泉分布的差异无统计学意义(P>0.05)。运气相合方面,出生于天刑、小逆、不和之年的慢性萎缩性胃炎患者比例最高,出生于岁会、同岁会之年的比例最低,差异有统计学意义(P<0.05)。【结论】慢性萎缩性胃炎患者的先天运气禀赋以肝郁气滞、燥、寒为主要特点,治疗上当从肝论治,以疏肝和胃为法,辅以滋阴润燥、顾护脾胃之阳气、针药并用。Objective To explore the distribution of circuit-qi indicators at birth in the patients with chronic atrophic gastritis(CAG)based on five-circuit and six-qi theory,and to summarize their congenital circuit-qi endowment features,so as to explore treatment strategies for CAG accordingly.Methods From January 2018 to December 2023,a total of 2046 cases of patients with CAG admitted to Shenzhen Bao an Traditional Chinese Medicine Hospital Affiliated to Guangzhou University of Chinese Medicine(i.e.,Shenzhen Bao an Traditional Chinese Medicine Hospital)were selected,and the distribution of the five-circuit and six-qi indicators at birth in the patients was analyzed by using the SPSS 24.0 statistical software.The features of the congenital circuit-qi endowment were summarized,and then the corresponding treatment strategies were put forward.Results The CAG patients had the highest proportions of circuit-qi endowment features at birth,showing as the yearly circuit being deficiency fire circuit,the dominant circuit being metal circuit,the dominant qi being yangming dryness-gold,the guest qi being taiyang cold-water,and sitian being shaoyang minister fire and zaiquan being jueyin wind-wood,and the differences in the distribution of the dominant circuit,dominant qi,and guest qi were statistically significant(P<0.05 or P<0.01),but differences in the distribution of the yearly circuit and sitian/zaiquan were not statistically significant(P>0.05).The analysis of the combined circuit-qi features at birth showed that the proportion of patients with CAG born in the year of celestial restriction,mildly-rebellious year and disharmony year was the highest,and the proportion of those born in the convergent year and iso-convergent year was the lowest,with statistically significant differences(P<0.05).Conclusion The congenital circuit-qi endowment of patients with CAG is characterized by liver depression and qi stagnation,dryness and coldness.Therefore,the treatment for CAG should be from the perspective of the liver,with the method of s
