耳内镜下外耳道清理后碘酊烧灼联合曲安奈德益康唑乳膏治疗真菌性外耳道炎的临床效果

摘　　要：目的分析真菌性外耳道炎采用曲安奈德益康唑乳膏联合碘酊烧灼的临床效果。方法选取2023年6月至2024年3月80例在东莞市滨海湾中心医院耳鼻咽喉头颈外科治疗的真菌性外耳道炎患者,随机分为对照组和试验组,每组各40例,根据单双耳真菌性外耳道炎患病情况,纳入研究的总耳例数为128例。对照组使用曲安奈德益康唑乳膏,试验组在对照组基础上加用碘酊烧灼治疗。观察两组的治疗有效率、复发率,比较两组治疗前后的症状评分、听力功能指标以及血清降钙素原(PCT)、C反应蛋白(CRP)和红细胞沉降率水平变化。结果治疗后两组患者症状评分、听力功能指标、血清PCT、CRP水平与红细胞沉降率均降低,且试验组低于对照组(P<0.05)。试验组的治愈率和总有效率均高于对照组(P<0.05);试验组治疗后1个月复发率显著低于对照组(P<0.05),两组均无明显不良反应。结论经外耳道清理后,真菌性外耳道炎使用曲安奈德益康唑乳膏与碘酊烧灼共同治疗能够有效改善其症状,并且复发率低,安全性良好。Objective To analyze the efficacy of triamcinolone acetonide and econazole cream combined with iodine tincture cauterization in the treatment of fungal otitis externa.Methods A total of 80 patients with fungal otitis externa admitted to the Department of Otolaryngology Head and Neck Surgery of the Dongguan Marina Bay Central Hospital from June 2023 to March 2024 were prospectively selected and they were randomly divided into the control group(n=40)and the experimental group(n=40).According to the prevalence of fungal otitis externa in both ears,the total number of ear cases included in the study was 128.The control group was treated with triamcinolone acetonide and econazole cream,while the experimental group was treated with iodine tincture cauterization on the basis of the control group.The treatment effective rate and relapse rate of the two groups were observed.Before and after treatment,the level changes of symptom score,hearing function index,serum procalcitonin(PCT),C-reactive protein(CRP)and erythrocyte sedimentation rate were compared between the two groups.Results After treatment,the symptom score,hearing function index,serum factors level and erythrocyte sedimentation rate of the two groups of patients all decreased,and the experimental group was lower than that of the control group,with statistically significant differences(P<0.05).The cure rate and total effective rate of the experimental group were all higher than those of the control group,with statistically significant differences(P<0.05).One month after treatment,the relapse rate of the experimental group was obviously lower than that of the control group,with statistically significant difference(P<0.05),and there were no obvious adverse reactions in either group.Conclusion After external auditory canal cleaning,the treatment of fungal otitis externa with triamcinolone acetonide and econazole cream combined with iodine tincture cauterization can effectively improve its symptoms,with low relapse rate and good safety.
