摘 要:高原性高血压是指人员在高原地区时血压升高的现象,其特征为舒张压明显上升,且通常在返回平原后血压恢复正常。初次进入高原地区的人群通常会经历血压升高和其他高原反应,但一般不会出现心功能不全或肺动脉高压等严重症状。本研究通过对1名健身人员在平原与高原地区心功能显著差异进行案例分析,旨在为未来高原官兵进驻高原前筛选高风险人群提供理论依据。2023年11月,某部官兵(驻地海拔500 m)在轮换高原任务期间,体检发现其中1例在平原地区和高原地区的2次体检中心功能表现差异性巨大。该患者既往有健身锻炼史,曾使用药物增肌,可能导致心肌损伤和心功能下降。然而,鉴于患者锻炼时间较短且为青壮年,心功能方面尚未出现明显异常。由于高原环境的特殊性,可能使患者心功能损害显现,这可能是导致患者在平原地区和高原地区心功能表现出显著差异的原因之一。本研究通过详细的医学检查和数据分析,为健身人员如何在不同海拔环境下调整其心脏功能提供了深入见解,并为有健身活动史人员的心脏健康管理提供了重要的理论依据。本研究对未来高原官兵在高原地区部署前进行高风险心血管疾病筛查提供了实践参考。High-altitude hypertension is a condition in which a person’s blood pressure rises while in plateau,characterized by a significant increase in diastolic blood pressure that usually returns to normal upon return to the plain areas.People who enter plateau for the first time usually experience elevated blood pressure and other altitude sickness,but do not manifest serio us symptoms such as cardiac insufficiency or pulmonary hypertension.This study aims to provide a theoretical basis for the future screening of high-risk groups among plateau soldiers before their deployment to high-altitude areas,through a case analysis of one fitness individual showing significant differences in cardiac function between plain and plateau regions.During the rotation of officers and soldiers of a certain troop(stationed at an altitude of 500 meters)to plateau duty in November 2023,the physical examination revealed that one case had a huge variability in the performance of cardiac function through two physical examinations in plain and plateau areas.The patient’s previous history of fitness exercise and medication to increase muscle mass may lead to myocardial injury and decreased cardiac function.However,given that the patient exercised for a relatively short period of time and was a young adult,no significant abnormalities in cardiac function have been observed.The specificity of the plateau environment may have made the impairment of cardiac function vis ible in the patient,which m ay be one of the reas ons for the s ignificant difference in cardiac function demonstrated by the patient in plain and plateau areas.This study provides ins ights into unders tanding how fitness pers onnel adjust their cardiac function in environm ents with different altitudest hrough detailed me dical examinations and data analysis,and offers an important theoretical basis for cardiac health management of personnel with a history of fitness activities.This study also provides a practical reference for future screening of high-risk cardiovascular dis
