坤泰胶囊联合炔雌醇环丙孕酮片对PCOS患者激素水平与周期排卵率的影响

摘　　要：目的分析坤泰胶囊联合炔雌醇环丙孕酮片对多囊卵巢综合征(PCOS)患者激素水平与周期排卵率的影响。方法回顾性选取2022年6月—2023年5月湛江市妇幼保健院收治的PCOS患者120例,根据用药方案不同分为中西联合组60例和西药组60例。西药组采用炔雌醇环丙孕酮片治疗,中西联合组在西药组基础上加用坤泰胶囊治疗。于治疗前和治疗9个月后比较2组激素水平,周期排卵率、妊娠率及不良反应。结果治疗9个月后,2组血清睾酮、黄体生成素、卵泡刺激素及催乳素水平较治疗前降低,血清性激素结合球蛋白水平较治疗前升高,且中西联合组降低/升高幅度大于西药组(P<0.05或P<0.01);中西联合组周期排卵率和妊娠率均高于西药组(75.00%vs.50.00%,χ^(2)/P=8.000/0.005;71.67%vs.46.67%,χ^(2)/P=7.761/0.005);中西联合组与西药组不良反应发生率比较,差异无统计学意义(8.33%vs.10.00%,χ^(2)=0.100,P=0.752)。结论坤泰胶囊联合炔雌醇环丙孕酮片治疗PCOS,可有效改善激素水平,提高周期排卵率和妊娠率,且安全性较高。Objective To analyse the effect of Kuntai capsules combined with ethinylestradiol and cyproterone acetate tablets on hormone levels and cycle ovulation rate in patients with PCOS.Methods A total of 120 patients with PCOS admitted to Zhanjiang Maternal and Child Health Hospital from June 2022 to May 2023 were retrospectively selected,and they were divided into the Chinese and western medicine group and the western medicine group,with 60 cases in each group.The western medicine group was treated with ethinylestradiol and cyproterone acetate tablets,and the Chinese and western medicine group was treated with Kuntai capsules on the basis of the western medicine group.Hormone levels before treatment and 9 months after treatment,cycle ovulation rate,pregnancy rate,and adverse reactions were compared between the two groups.Results After 9 months of treatment,the serum levels of T,LH,FSH and PRL in the two groups decreased compared with those before treatment,and the serum level of SHBG increased compared with that before treatment,and the decrease/increase in the Chinese and western medicine group were greater than those in the western medicine group(P<0.05 or P<0.01).The cycle ovulation rate and pregnancy rate in the Chinese and western medicine group were higher than those in the western medicine group(75.00%vs.50.00%,χ^(2)/P=8.000/0.005;71.67%vs.46.67%,χ^(2)/P=7.761/0.005).There was no significant difference in the incidence of adverse reactions between the Chinese and western medicine group and the western medicine group(8.33%vs.10.00%,χ^(2)=0.100,P=0.752).Conclusion Kuntai capsules combined with ethinylestradiol and cyproterone acetate tablets in the treatment of PCOS can effectively improve hormone levels,increase cycle ovulation rate and pregnancy rate,and with higher safety.
