中药降糖膏对糖尿病肾病患者尿微量白蛋白的影响及肾功能的保护作用

摘　　要：目的研究中药降糖膏对糖尿病肾病(DN)患者尿微量白蛋白(U-MA)的影响及肾功能保护作用。方法选取2023年3月-2024年6月在我院接受治疗的DN患者100例,随机分为对照组和研究组,各50例,2组均接受糖尿病基础治疗。对照组给予厄贝沙坦片,研究组在此基础上给予中药降糖膏治疗。治疗30天后,评估2组临床疗效、中医证候变化、血糖及肾功能相关指标的改善情况。结果治疗后,研究组总有效率高于对照组(P<0.05),且治疗后研究组的空腹血糖(FBG)、糖化血红蛋白(HbAlc)、血尿素氮(BUN)、血清肌酐(SCr)和U-MA水平均低于对照组(P<0.05);治疗后,2组中医证候积分水平下降,且研究组低于对照组(P<0.05);2组经过治疗后,在不良反应发生情况方面比较,差异无统计学意义(P>0.05)。结论中药降糖膏对于DN治疗效果确切,可调节血糖、降低U-MA水平、维持肾功能稳定,值得临床推广应用。Objective To study the effect of Chinese herbal blood sugar-lowering cream on urinary microalbumin(U-MA)in patients with diabetic nephropathy(DN)and its protective effect on renal function.Methods 100 patients with DN who were treated in Suzhou Ninth People's Hospital from March 2023 to June 2024 were randomly divided into the control group and the study group,with 50 cases in each group.Both groups received basic treatment for diabetes.The control group was treated with Irbesartan tablets,and the study group was treated with Chinese herbal blood sugar-lowering cream on the basis of the treatment of the control group.After 30 days of treatment,the clinical efficacy,traditional Chinese medicine(TCM)syndromes changes,improvement of blood glucose and kidney function related indicators of two groups were evaluated.Results After treatment,the total effective rate of the study group was higher than that of the control group(P<0.05),and the levels of fasting blood glucose(FBG),glycated hemoglobin(HbAlc),blood urea nitrogen(BUN),serum creatinine(SCr),and U-MA in the study group were lower than those in the control group after treatment(P<0.05).After treatment,the scores of TCM syndrome in both groups decreased,and the study group was lower than the control group(P<0.05).After treatment,there was no statistically significant difference in the incidence of adverse reactions between two groups(P>0.05).Conclusion Chinese herbal blood sugar-lowering cream has a definite therapeutic effect on DN,which can regulate blood sugar level,reduce U-MA level and maintain the stability of renal function.It is worthy of clinical promotion and application.
