杜氏理筋手法联合中药熏洗对老年膝关节内侧单髁置换术后疼痛及功能恢复的影响

摘　　要：目的 探讨杜氏理筋手法联合中药熏洗治疗对老年膝关节内侧单髁置换术(UKA)后功能恢复的影响。方法 选取2021年4月-2024年6月于我院进行膝关节内侧UKA的102例老年患者,按照随机数字表法分为对照组与观察组,各51例。2组术后均接受杜氏理筋手法治疗,观察组另联合艾香舒筋洗药治疗,共治疗4周。分析2组疼痛程度、膝关节功能评分(HSS)、膝关节肿胀度及膝关节活动度(ROM)变化,另统计不良反应发生状况。结果 治疗2、3、4周后,2组视觉模拟评分法(VAS)和髌骨上、下缘膝关节肿胀度均呈下降趋势,且观察组治疗3、4周后VAS评分和髌骨上、下缘膝关节肿胀度均显著低于对照组(P<0.05)。治疗2、3、4周后,2组HSS评分、ROM均呈上升趋势,且观察组治疗3、4周后HSS评分、ROM均高于对照组(P<0.05)。结论 杜氏理筋法联合中药熏洗对老年膝关节内侧UKA术后患者安全有效,减轻术后肿胀、疼痛状况,改善膝关节功能。Objective To investigate the effect of Du's tendon-regulation manipulation combined with fumigation and washing with traditional Chinese medicine on functional recovery in the elderly after medial unicompartmental knee arthroplasty(UKA).Methods A total of 102 elderly patients who underwent medial UKA in the hospital from April 2021 to June 2024 were selected and divided into the control group and the observation group,according to random number table method,with 51 cases in each group.Both groups were treated with Du's tendon-regulation manipulation after surgery,and the observation group was additionally treated with Aixiang Shujin Xiyao.All patients were given 4 weeks of treatment.Changes in pain degree,Hospital for Special Surgery(HSS),and knee range of motion(ROM)in two groups were analyzed.The occurrence of adverse reactions was recorded.Results After 2,3,and 4 weeks of treatment,Visual Analogue Scale(VAS)scores and upper and lower patellar knee swelling showed a decreasing trend in both groups,and VAS scores and upper and lower patellar knee swelling in the observation group were significantly lower than those in the control group after 3 and 4 weeks of treatment(P<0.05).After 2,3,and 4 weeks of treatment,HHS scores and ROM showed an increasing trend in both groups,and HHS scores and ROM in the observation group were higher than those in the control group after 3 and 4 weeks of treatment(P<0.05).Conclusion Du's tendon-regulation manipulation combined with fumigation and washing with traditional Chinese medicine is safe and effective for elderly patients undergoing medial UKA.It can alleviate postoperative swelling and pain,and improve knee function.
