介入联合宫腔镜治疗剖宫产瘢痕处妊娠的效果分析

摘　　要：目的探讨介入联合宫腔镜治疗剖宫产瘢痕处妊娠(CSP)的效果分析。方法采用回顾性分析方法,选取我院2022年1月至2023年12月收治的CSP患者60例,分为介入联合宫腔镜组(观察组,n=30)和传统治疗组(对照组,n=30)。对比两组的治疗成功率、各项治疗指标、并发症及生育情况。结果观察组的治疗成功率为96.67%(29/30),显著高于对照组的73.33%(22/30)(P<0.05);观察组的平均出血量、平均住院时间、子宫恢复正常时间均少于对照组(P<0.05);观察组的并发症发生率为6.67%(2/30),包括1例轻度发热和1例轻微腹痛;对照组的并发症发生率为26.67%(8/30),包括3例子宫穿孔、2例大出血及3例感染(P<0.05);随访12个月,观察组中有11例患者成功妊娠,妊娠率为36.67%;对照组中仅有3例患者成功妊娠,妊娠率为10.00%(P<0.05)。结论介入联合宫腔镜治疗CSP的效果显著,具有治疗成功率高、术中出血少、住院时间短及并发症少等优势,且能保留生育功能。Objective To explore the effect analysis of interventional combined with hysteroscopy in the treatment of cesarean scar pregnancy(CSP).Method A retrospective analysis was conducted on 60 patients with CSP admitted to our hospital from January 2022 to December 2023.They were divided into an intervention combined with hysteroscopy group(observation group,n=30)and a traditional treatment group(control group,n=30).Compare the treatment success rate,various treatment indicators,complications,and fertility status between the two groups.Result The treatment success rate of the observation group was 96.67%(29/30),significantly higher than the control group's 73.33%(22/30)(P<0.05);The average amount of bleeding,average length of hospital stay,and time for uterine recovery in the observation group were all less than those in the control group(P<0.05);The incidence of complications in the observation group was 6.67%(2/30),including 1 case of mild fever and 1 case of mild abdominal pain;The incidence of complications in the control group was 26.67%(8/30),including 3 cases of uterine perforation,2 cases of severe bleeding,and 3 cases of infection(P<0.05);During a 12-month follow-up,11 patients in the observation group successfully became pregnant,with a pregnancy rate of 36.67%;Only 3 patients in the control group successfully became pregnant,with a pregnancy rate of 10.00%(P<0.05).Conclusion Intervention combined with hysteroscopy is effective in treating CSP,with advantages such as high treatment success rate,less intraoperative bleeding,shorter hospital stay,and fewer complications,while preserving fertility.
