深圳市南山区严重精神障碍患者危险行为发生情况及影响因素分析

摘　　要：目的分析深圳市南山区严重精神障碍患者危险行为发生情况及影响因素,为提升严重精神障碍患者管理服务质量和预防危险行为的发生提供依据。方法收集2003年1月至2024年5月深圳市精神卫生防治工作信息管理系统中南山区在册严重精神障碍患者资料,采用χ^(2)检验、Spearman相关分析和Logistic回归分析危险行为发生的有关影响因素。结果共调查患者3614例,发生危险行为患者299例,危险行为发生率为8.27%。多因素Logistic回归分析显示,男性(OR=1.771,95%CI:1.351~2.321)、弱监护(OR=1.793,95%CI:1.107~2.904)、不规律服药(OR=2.100,95%CI:1.174~3.766)、既往住院次数(OR1次=18.479,95%CI:11.635~29.350;OR2次=20.390,95%CI:12.014~34.608;OR3次及以上=48.896,95%CI:30.215~79.126)是严重精神障碍患者危险行为发生的危险因素。结论南山区严重精神障碍患者危险行为发生率较高,男性、弱监护、不规律服药和既往多次住院患者发生危险行为的风险较高,应对其进行密切关注和加强管理。Objective To analyze the occurrence and influencing factors of risky behaviors among patients with severe mental disorders in Nanshan district,Shenzhen,and to provide a basis for improving the quality of management services for these patients.Methods Collected data on registered patients with the mental health information management system in Nanshan district were retrieved,and the chi-square test,Spearman correlation analysis,and logistic regression analysis were used to identify factors that influenced the occurrence of risky behaviors.Results A total of 3614 patients were investigated,299 of them engaged in risky behaviors,with a risky behavior incidence of 8.27%.Multivariate logistic regression analysis showed that male(OR=1.771,95%CI:1.351-2.321),weak monitoring(OR=1.793,95%CI:1.107-2.904),irregular medication(OR=2.100,95%CI:1.174-3.766),and previous hospitalization history(OR_1=18.479,95%CI:11.635-29.350,OR_2=20.390,95%CI:12.014-34.608,OR_(3 times or more)=48.896,95%CI:30.215-79.126)were risk factors for these patients.Conclusion The incidence of risky behavior was relatively high among patients with severe mental disorders in the Nanshan district.Male patients who had weak supervision,took medication irregularly,and had been hospitalized multiple times were at higher risk of engaging in risky behaviors.Therefore,close attention and strengthened management should be paid to them.
