CT联合MP-DNA及血清CRP、PCT检测在小儿重症支原体肺炎中的诊断及评估中的应用分析

摘  要:探讨计算机断层扫描(CT)联合肺炎支原体(MP)DNA及血清C反应蛋白(CRP)、降钙素原(PCT)检测在小儿重症支原体肺炎(SMPP)中的诊断及评估中的应用。选取2023年2月—2024年5月医院收治的98例支原体肺炎(MPP)患儿作为研究对象,根据患儿病情严重程度分为重症组(25例)和轻症组(73例),比较两组CT影像特征及MP-DNA、血清CRP、PCT水平,并分析CT联合MP-DNA及血清CRP、PCT对小儿重症支原体肺炎的诊断及预后评估价值。重症组大片实变影占比、MP-DNA高载量占比、CRP、PCT水平高于轻症组(P<0.05);ROC曲线显示,CT、MP-DNA、CRP、PCT联合对小儿SMPP诊断的AUC为0.969,高于CT的0.710、MP-DNA的0.762、CRP的0.725、PCT的0.778(Z=3.140;Z=3.809;Z=4.356;Z=5.213,P<0.05);预后不良组大片实变影占比、MP-DNA高载量占比、CRP、PCT水平高于预后良好组(P<0.05);ROC曲线显示,CT、MP-DNA、CRP、PCT联合预测小儿MPP预后的AUC为0.921,高于CT的0.729、MP-DNA的0.711、CRP的0.764、PCT的0.729(Z=4.227;Z=3.454;Z=2.728;Z=2.582,P<0.05)。CT、MP-DNA、CRP、PCT对SMPP具有显著的诊断价值,临床可用于病情诊断和预后评估。To explore the application of computed tomography(CT)combined with mycoplasma pneumoniae(MP)DNA,serum Creactive protein(CRP)and procalcitonin(PCT)in the diagnosis and evaluation of severe mycoplasma pneumoniae pneumonia(SMPP)in children,a total of 98 children with mycoplasma pneumoniae pneumonia(MPP)were enrolled as the research objects between February 2023 and May 2024.According to disease severity,they were divided into severe group(25 cases)and mild group(73 cases).CT imaging characteristics,levels of MP-DNA,serum CRP and PCT in the two groups were compared.The value of CT combined with MP-DNA,serum CRP and PCT in the diagnosis and prognosis evaluation of SMPP was analyzed.The proportions of large consolidation shadow and MP-DNA high load,levels of CRP and PCT in severe group were higher than those in mild group(P<0.05).ROC curves analysis showed that AUC of CT combined with MP-DNA,CRP and PCT in the diagnosis of SMPP was 0.969,greater than that of single index(0.710,0.762,0.725,0.778;Z=3.140;Z=3.809;Z=4.356;Z=5.213,P<0.05).The proportions of large consolidation shadow and MP-DNA high load,levels of CRP and PCT in poor prognosis group were higher than those in good prognosis group(P<0.05).ROC curves analysis showed that AUC of CT combined with MP-DNA,CRP and PCT for predicting prognosis of MPP was 0.921,greater than that of single index(0.729,0.711,0.764,0.729;Z=4.227;Z=3.454;Z=2.728;Z=2.582,P<0.05).The diagnostic value of CT,MP-DNA,CRP and PCT is significant in SMPP,which can be applied in disease diagnosis and prognosis evaluation clinically.
