静脉注射人免疫球蛋白联合注射用哌拉西林钠他唑巴坦钠治疗新生儿败血症的临床效果

摘　　要：目的本研究旨在探讨静脉注射人免疫球蛋白(IVIG)联合注射用哌拉西林钠他唑巴坦钠治疗新生儿败血症的临床效果。方法本研究选取2021年1月至2023年12月临沂市第三人民医院新生儿重症监护室收治的100例新生儿败血症患者作为研究对象。采用随机数字表法将患者随机分为50例对照组和50例治疗组。两组接受常规治疗方法,对照组给予注射用哌拉西林钠他唑巴坦钠,治疗组在对照组的基础上给予静脉注射人免疫球蛋白。分析两组住院时间、病情缓解情况、炎症指标变化、免疫功能、肝肾功能、不良反应发生率。结果治疗组住院时间短于对照组(P<0.05)。第7天,治疗组血培养阳性转化率为94%,优于对照组(76%)(P<0.05)。联合治疗后,治疗组C反应蛋白、白细胞计数、降钙素原、白细胞介素6、丙氨酸氨基转移酶、丙氨酸氨基转换酶和血清肌酐水平均低于对照组(均P<0.05)。治疗组免疫球蛋白G、免疫球蛋白A、免疫球蛋白M水平较对照组更高(均P<0.05)。治疗组不良反应发生率低于对照组(P<0.05)。结论IVIG联合注射用哌拉西林钠他唑巴坦钠治疗新生儿败血症能提高临床治疗效果,推动病情的快速缓解。Objective The aim of this study is to investigate the clinical efficacy of intravenous injection of human immunoglobulin(IVIG)combined with injection of piperacillin sodium and tazobactam sodium in the treatment of neonatal sepsis.Methods This study selected 100 neonatal sepsis patients admitted to the neonatal intensive care unit of Linyi Third People's Hospital from January 2021 to December 2023 as the research subjects.Patients were randomly divided into a control group of 50 cases and a treatment group of 50 cases using a random number table method.The two groups received conventional treatment,the control group was given piperacillin sodium for injection and tazobactam sodium,and the treatment group was given intravenous human immunoglobulin based on the control group.Analyze the hospitalization time,disease relief,changes in inflammatory indicators,immune function,liver and kidney function,and incidence of adverse reactions between two groups.Results The hospitalization time of the treatment group was shorter than that of the control group(P<0.05).On the 7th day,the positive conversion rate of blood culture in the treatment group was 94%,which was better than that in the control group(76%)(P<0.05).After combined treatment,the levels of C-reactive protein,white blood cell count,procalcitonin,interleukin-6,alanine aminotransferase,alanine aminotransferase,and serum creatinine in the treatment group were all lower than those in the control group(P<0.05);The levels of immunoglobulin G,immunoglobulin M,and immunoglobulin A in the treatment group were higher than those in the control group(P<0.05).The incidence of adverse reactions in the treatment group was lower than that in the control group(P<0.05).Conclusions The combination of IVIG and injectable piperacillin sodium and tazobactam sodium can improve clinical efficacy and promote rapid remission of neonatal sepsis.
