摘 要:目的 分析枸橼酸咖啡因联合牛肺表面活性剂治疗新生儿呼吸窘迫综合征的应用价值。方法 选取连云港市赣榆区人民医院2021年1月至2024年8月收治的82例呼吸窘迫综合征新生儿,随机数字表法分为两组,各41例,对照组静脉滴注枸橼酸咖啡因注射液,研究组在对照组基础上联合注射用牛肺表面活性剂,对比血气指标[动脉血氧分压(PaO_(2))、动脉血氧饱和度(SaO_(2))、动脉血二氧化碳分压(PaCO_(2))]、炎症因子(C反应蛋白、白细胞介素6和肿瘤坏死因子α)、临床指标(呼吸机使用时间及住院时间)、并发症发生率。结果 治疗后,研究组PaO_(2)、SaO_(2)高于对照组,PaCO_(2)及炎症因子水平均低于对照组(均P<0.05);研究组呼吸机使用时间及住院时间均短于对照组(均P<0.05);两组并发症发生率对比,P>0.05。结论 枸橼酸咖啡因联合牛肺表面活性剂治疗呼吸窘迫综合征新生儿能够显著改善血气指标,降低炎症因子水平,促进患儿康复,不会增加并发症发生风险。Objective To analyze the application value of caffeine citrate combined with bovine pulmonary surfactant in the treatment of neonatal respiratory distress syndrome.Methods A total of 82 neonates with respiratory distress syndrome admitted to Ganyu District People’s Hospital of Lianyungang City from January 2021 to August 2024 were selected,and the random number table was divided into two groups,41 cases in each group,the control group was injected with intravenous caffeine citrate injection,and the study group was combined with the injection of bovine lung surfactant on the basis of the control group,and the blood gas indexes[arterial blood oxygen partial pressure(PaO_(2)),arterial oxygen saturation(SaO_(2)),arterial blood carbon dioxide partial pressure(PaCO_(2))],inflammatory factors[C-reactive protein,interleukin-6 and tumor necrosis factorα],clinical indicators[duration of ventilator use and length of hospital stay],complication rate.Results After treatment,the levels of PaO_(2) and SaO_(2) in the study group were higher than those in the control group,and the levels of PaCO_(2) and inflammatory factors were lower than those in the control group(all P<0.05),and the duration of ventilator use and hospital stay in the study group were shorter than those in the control group(all P<0.05),and the complication rate of the two groups was compared,P>0.05.Conclusions Caffeine citrate combined with bovine pulmonary surfactant in the treatment of neonatal respiratory distress syndrome can significantly improve blood gas indexes,reduce the level of inflammatory factors,promote the recovery of children,without increasing the risk of complications.
