扁桃体切除术患儿家属心理压力评估与相关因素分析

摘　　要：目的 探究扁桃体切除手术患儿的家属的心理压力水平及相关影响因素。方法 采用横断面研究,收集2022年1月~2024年1月本院收治的106例行扁桃体切除手术的患儿家属资料,通过问卷与量表统计患儿家属的心理压力[中文版知觉压力量表(CPSS)]、一般资料(年龄、性别、受教育程度、家庭人均收入及婚姻状况)、患儿疾病及治疗情况(患儿疾病类型、手术时间、呼吸恢复时间、拔管时间、住院时间及并发症发生情况等)、心理及社会因素(疾病知识掌握程度、心理弹性及护理满意度),分析患儿家属的心理压力水平及相关影响因素。结果 共收集106例患儿家属资料,CPSS评分为34.55±6.87分。不同住院时间、并发症发生情况及心理和社会因素的患儿家属,其CPSS评分存在统计学显著差异(P<0.05)。将这些因素纳入多因素分析后,结果表明,患儿的住院时间、并发症发生情况、家属对疾病知识的掌握程度及护理满意度均为影响CPSS评分的独立危险因素。其中,家属的护理满意度和疾病知识掌握程度的回归系数较高,对CPSS评分的影响显著。结论 行扁桃体切除手术患儿的家属心理压力普遍较大,临床中应予以重视,患儿住院时间、并发症发生情况、家属疾病知识掌握程度及护理满意度均显著影响家属心理压力水平,其中家属护理满意度与疾病知识掌握程度是影响家属心理压力的重要因素,临床医师及护理人员应引起重视。Objective To explore the psychological stress level and related influencing factors of family members of children undergoing tonsillectomy.Methods A cross-sectional study was conducted to collect the data of 106 family members of children who underwent tonsillectomy in our hospital from January 2022 to January 2024.The psychological stress[Chinese version of perceived stress scale(CPSS)],general data(age,gender,education level,family per capita income and marital status),disease and treatment of children(disease type,operation time,respiratory recovery time,extubation time,hospitalization time and complications),psychological and social factors(disease knowledge mastery,resilience and nursing satisfaction)were counted by questionnaires and scales.The psychological stress level and related influencing factors of children's family members were analyzed.Results A total of 106 family members were collected,and the CPSS score was 34.55±6.87.There were significant differences in CPSS scores among family members of children with different hospitalization time,complications,psychological and social factors(P<0.05).The above factors were included in the multivariate analysis.It was shown that hospitalization time,complications,family members'knowledge of disease and nursing satisfaction were independent risk factors affecting CPSS score.Among them,the regression coefficient between the disease knowledge mastery and the satisfaction of family care was higher,which had a greater impact on the CPSS score.Conclusion The psychological pressure of family members of children undergoing tonsillectomy is generally high,which should be paid attention to in clinical practice.The hospitalization time of children,the occurrence of complications,the mastery of disease knowledge of family members and nursing satisfaction all significantly affect the psychological pressure level of family members.Among them,the disease knowledge mastery and the nursing satisfaction of family members are important factors affecting the psychological pres
