影响试管婴儿妊娠成功率的不孕症患者心理因素分析

摘　　要：目的 分析影响试管婴儿妊娠成功率的不孕症患者心理因素,特别关注女性身份认同、家庭关系及其对生育动力的影响。方法 本研究对象为337名2022年7月~2023年6月在四川锦欣西囡妇女儿童医疗中心生殖中心进行试管婴儿治疗的不孕症患者。采用问卷调查法收集患者一般资料、心理状况及家庭关系信息。使用生活事件量表(LESPW)、医院焦虑抑郁量表(HADS)、女性身份认同量表和家庭关系评估量表进行心理评估。通过多因素Logistic回归分析妊娠成功率的影响因素。结果 妊娠成功率为50.74%。多因素Logistic回归分析显示,年龄(<30岁,OR=1.774,95%CI:1.509~2.347)、不孕年限(≤3年,OR=1.440,95%CI:0.927~1.625)为试管婴儿妊娠成功率的保护因素。生活事件应激(得分>200分,OR=1.277,95%CI:1.014~2.457)、焦虑(OR=1.352,95%CI:1.103~1.657)和抑郁(OR=1.289,95%CI:1.056~1.573)为危险因素。此外,女性身份认同水平(OR=0.823,95%CI:0.687-0.986)和与母亲的关系质量(OR=0.856,95%CI:0.723~1.013)与妊娠成功率呈正相关。结论 试管婴儿妊娠成功率受多种心理因素影响,包括生活事件应激、情绪状态、女性身份认同和家庭关系。建议在试管婴儿治疗过程中加强心理评估和干预,特别关注女性身份认同和家庭关系对生育动力的影响,以提高妊娠成功率。Objective To analyze the psychological factors affecting the success rate of in vitro fertilization in infertile patients,with special attention to female identity,family relationship and their influence on fertility motivation.Methods The subjects of this study were 337 infertility patients who underwent IVF treatment at the Reproductive Center of Jinxin Xinan Women and Children Medical Center,Sichuan Province from July 2022 to June 2023.General data,psychological status and family relationship information of patients were collected by questionnaire.life events scale for pregnant women(LESPW),hospital anxiety and depression scale(HADS),feminist identity development scale and family relationship assessment scale were used for psychological assessment.The influencing factors of pregnancy success rate were analyzed by multiple Logistic regression.Results The pregnancy success rate was 50.74%.Multivariate Logistic regression analysis showed that age(<30 years old,OR=1.774,95%CI:1.509~2.347)and infertility years(≤3 years,OR=1.440,95%CI:0.927~1.625)were protective factors for the success rate of pregnancy in IVF.Life event stress(score>200,OR=1.277,95%CI:1.014~2.457),anxiety(OR=1.352,95%CI:1.103~1.657)and depression(OR=1.289,95%CI:1.056~1.573)were risk factors.In addition,the level of female identity(OR=0.823,95%CI:0.687~0.986)and the quality of the relationship with the mother(OR=0.856,95%CI:0.723~1.013)were positively correlated with pregnancy success.Conclusion The success rate of IVF pregnancy is affected by many psychological factors,including life event stress,emotional state,female identity and family relationship.It is suggested that psychological evaluation and intervention should be strengthened during IVF treatment,with special attention to the influence of female identity and family relationship on fertility motivation,in order to improve the success rate of pregnancy.
