慢性阻塞性肺疾病患者家属心理压力评估与影响因素分析

摘　　要：目的 探究慢性阻塞性肺疾病(COPD)患者家属心理压力水平及相关影响因素分析。方法 本项横向研究在2022年1月~2024年5月期间,搜集了本院收治的113名慢性阻塞性肺病(COPD)患者及其家属的数据。通过量表和问卷调查的方式,评估了患者一级亲属的心理负担,并进行了相关性分析,以识别可能影响家属心理压力的各种因素。结果 (1)分析结果表明,113位患者家属的心理压力评分(CPSS)为31.61±8.28。在比较不同人均收入水平的患者家属、不同呼吸困难指数的患者家属、不同心理弹性、不同疾病知识掌握程度以及不同护理满意度的患者家属时,发现CPSS评分的统计学差异显著(P<0.05);(2)研究揭示,患者家属的心理压力评分与家庭人均收入、患者的呼吸困难指数、家属的心理弹性、对疾病知识的掌握程度以及护理满意度存在显著的相关性(P<0.05)。(3)进一步分析确认,家庭人均收入、患者的呼吸困难指数、家属的心理弹性以及护理满意度均为显著影响患者家属CPSS评分的因素。结论 COPD患者家属心理压力普遍较大,临床中为COPD患者诊疗时,应注意及时关注并调节患者家属的心理状态。家庭人均收入、患者的呼吸困难指数、家属的心理弹性以及护理满意度都是影响患者家属CPSS评分的重要因素。因此,在临床实践中,应特别关注提升家属的护理满意度,以助力患者的病情恢复,减轻家属的心理压力。Objective To explore the level of psychological stress in family members of patients with chronic obstructive pulmonary disease(COPD)and analyze the related influencing factors.Methods This cross-sectional study collected data from 113 patients with chronic obstructive pulmonary disease(COPD)and their families who were admitted to our hospital between January 2022 and May 2024.By means of scale and questionnaire,the psychological burden of first-degree relatives of patients was assessed,and correlation analysis was conducted to identify various factors that may affect the psychological stress of family members.Results①The results showed that the psychological stress score of 113 family members was 31.61±8.28.When comparing family members of patients with different per capita income levels,family members of patients with different dyspnea index,family members of patients with different resilience,different disease knowledge and different nursing satisfaction,CPSS scores were found to have statistically significant differences(P<0.05).②The study revealed that the psychological stress score of the patients'family members was significantly correlated with the per capita income of the family,the dyspnea index of the patients,the resilience of the family members,the mastery of the disease knowledge and the satisfaction of nursing(P<0.05).③Further analysis confirmed that family per capita income,patients'dyspnea index,family members'resilience and nursing satisfaction were all factors that significantly affected the CPSS score of patients'family members.Conclusion The psychological stress of COPD patients'family members was generally large.In clinical diagnosis and treatment of COPD patients,timely attention should be paid to and adjustment of the psychological state of patients'family members.Family per capita income,patients'dyspnea index,family members'resilience and nursing satisfaction were all important factors affecting CPSS scores of patients'family members.Therefore,in clinical practice,special attention
