住院抑郁症患者痛苦表露现状及其影响因素分析

摘　　要：目的 调查住院抑郁症患者痛苦表露现状,并对其影响因素进行分析。方法 采用便利抽样的方法选取2024年1月~6月在江苏省扬州五台山医院住院治疗且诊断为抑郁症的患者207例作为研究对象,收集患者的社会人口学信息和痛苦表露指数问卷(DDI)得分,确定住院抑郁症患者痛苦表露的水平,并通过单因素分析和无序多分类Logistic回归分析不同痛苦表露水平的影响因素。结果 207例住院抑郁症患者的痛苦表露水平可以分为三组,分别为低水平组94例(45.41%)、中等水平组87例(42.03%)和高水平组26例(12.56%)。相较于高水平组,住院环境、正常的体质量指数(BMI)和住院期间是否接受无抽搐电休克治疗(MECT)是其他两组的共同影响因素;同时社会支持程度、已婚、有无躯体并发症、自知力与治疗态度水平低、抑郁程度轻是中等水平组的影响因素,女性、未婚、有自杀意念是低水平组的影响因素。结论 住院抑郁症患者痛苦表露状况不佳,临床工作人员可根据不同水平及其影响因素,有针对性地开展个性化的干预措施。Objective To investigate the present situation of distress disclosure in inpatients with depression and analyze the influencing factors of distress disclosure.Methods 207 patients who were hospitalized in Mount Wutai Hospital,Yangzhou,Jiangsu Province from January to June 2024 and diagnosed as depression were selected as the study subjects by convenient sampling method.The social demographic information and distress disclosure index(DDI)scores of the patients were collected to determine the distress disclosure level of the patients.Results The influencing factors of different levels of distress disclosure were analyzed by univariate analysis and disordered multiple Logistic regression.The distress disclosure level of 207 hospitalized patients with depression could be divided into three groups:low level group 94 cases(45.41%),medium level group 87 cases(42.03%)and high level group 26 cases(12.56%).Compared with the high level group,hospitalization environment,body mass index(BMI)and whether to receive modified electroconvulsive therapy(MECT)during hospitalization were the common influencing factors of the other two groups.Meanwhile,the degree of social support,marriage,physical complications,low level of self-awareness and treatment attitude,and mild depression were the influencing factors of the middle-level distress disclosure group,female,unmarried,and suicidal ideation were the influencing factors of the low-level distress disclosure group.Conclusion The distress disclosure level of hospitalized depression patients is low,and clinical nurses could carry out personalized intervention measures according to different levels and influencing factors.
