中国成人酒精使用障碍影响因素的Meta分析

摘　　要：目的 通过Meta分析探讨中国成人酒精使用障碍的影响因素。方法 计算机检索PubMed、Web of Science、Embase、Cochrane Library、中国生物医学文献数据库、中国知网、维普、万方数据库中有关于中国成人酒精使用障碍影响因素的相关文献,检索时间从建库~2024年3月31日。由2名研究人员独立筛选文献,并对纳入文献进行数据提取和质量评价,使用Stata 15.0软件进行分析。结果 最终纳入49篇文献,总样本量为18 697例。Meta分析结果显示,中国成人发生酒精使用障碍的主要影响因素包括:男性、中年、离异/丧偶、初中及以下学历、少数民族、农村、在业、家庭人均月收入低/高、工作地区在西部/北部、吸烟史、阳性家族史、情感压力大、家庭功能异常、经历家庭暴力、消极应对方式、焦虑、睡眠障碍、精神病史、躯体疾病、次饮酒量大、狂饮、同伴饮酒、家人赞同/中立饮酒态度以及积极饮酒期望。结论 一般因素(男性、中年、离异/丧偶、受教育程度、少数民族、农村、在业、家庭人均月收入、工作地区)、社会心理因素(吸烟史、阳性家族史、情感压力、家庭功能、经历家庭暴力、消极应对方式)、疾病相关因素(焦虑、睡眠障碍、精神病史、躯体疾病)和饮酒相关因素(次饮酒量、狂饮、同伴饮酒、家人对饮酒态度、积极饮酒期望)是中国成人酒精使用障碍的主要影响因素,其中家庭功能良好和家人反对饮酒为保护因素。Objective To explore the influencing factors of adult alcohol use disorders in China through Meta-analysis.Methods The literatures on the influencing factors of alcohol use disorder in Chinese were searched in PubMed,Web of Science,Embase,Cochrane Library,CBM,CNKI,VIP and Wanfang.The search period was from the establishment of the database to March 31,2024.The literature was screened independently by two researchers,and data extraction and quality evaluation were carried out for the included literature.Stata 15.0 software was used for analysis.Results 49 literatures were included,with a total sample size of 18697 cases.The results of Meta-analysis showed that the main influencing factors of alcohol use disorder among Chinese adults included:male,middle-aged,divorced/widowed,junior high school education or below,ethnic minorities,from rural areas,employed,low/high monthly family income per capita,work area in the west/north,smoking history,positive family history,high emotional stress,family function,experience of domestic violence,negative coping style,anxiety,sleep disorder,history of mental diseases,physical disease,heavy alcohol with each drink,binge drinking,peer drinking,family approval/neutral drinking attitude,positive drinking expectations.Conclusion Current general factors(male,middle age,divorced/widowed,education,ethnic minorities,rural area,employment,per capita monthly household income,work area),psychosocial factors(smoking history,positive family history,emotional stress,family functioning,experience of domestic violence,negative coping styles),disease factors(anxiety,sleep disorders,mental disorders,physical diseases)and alcohol factors(number of drinks,binge drinking,peer drinking,family attitude towards drinking,positive expectation of drinking)are the main influencing factors of adult alcohol use disorder in China,among which normal family function and family opposition to drinking are protective factors.
