摘　　要：目的 分析直肠癌永久性结肠造口(PC)患者病耻感现状及影响因素。方法 选取2021年8月~2024年7月期间本院收治的202例直肠癌PC患者为研究对象,采用自拟一般资料调查问卷、社会影响量表(SIS)、社会支持度评定量表(SSRS)、造口患者自我效能量表(SSES)对其进行调查。观察直肠癌PC患者的病耻感现状,并对直肠癌PC患者病耻感的影响因素进行单因素分析与多因素Logistic回归分析。结果 本组直肠癌PC患者SIS评分为65.16±3.72分,提示存在中重度的病耻感。经单因素与多因素Logistic回归分析显示,教育程度、家庭人均月收入、自理程度、造口时间、社会支持水平、自我效能水平是直肠癌PC患者病耻感的影响因素(P<0.05)。结论 直肠癌PC患者的病耻感处于中重度水平,影响因素主要与教育程度、家庭人均月收入、自理程度、造口时间、社会支持、自我效能有关,临床应加以重视,积极采取有效的心理干预措施,以减轻其病耻感,促进心理健康。Objective To analyze the current status and influencing factors of shame in patients with permanent colostomy(PC)for rectal cancer.Methods 202 patients with rectal cancer PC admitted to our hospital from August 2021 to July 2024 were selected as the observation subjects.A selfdesigned general information survey questionnaire,social impact scale(SIS),social support rating scale(SSRS),and self efficacy scale for ostomy patients(SSES)were used for the survey.Observe the current status of shame in patients with rectal cancer PC,and conduct univariate analysis and multivariate logistic regression analysis on the influencing factors of shame in patients with rectal cancer PC.Results The SIS score of PC patients with rectal cancer in this group was 65.16±3.72 points,indicating the presence of moderate to severe disease shame.Univariate and multivariate logistic regression analysis showed that education level,per capita monthly income of the family,self-care level,stoma time,social support level,and selfefficacy level were the influencing factors of shame in PC patients with rectal cancer(P<0.05).Conclusion The shame of PC patients with rectal cancer is at a moderate to severe level,and its influencing factors are mainly related to education level,per capita monthly income of the family,self-care level,stoma time,social support,and self-efficacy.Clinical attention should be paid to actively adopting effective psychological intervention measures to improve their shame and promote mental health.
