手外伤患者心理压力水平及影响因素分析

摘　　要：目的 探究手外伤患者心理压力水平及影响因素分析。方法 本研究采用横断面设计,利用便利抽样法收集了本院在2021年2月~2024年1月间接收的206例手外伤患者的临床资料。通过中文版知觉压力量表(CPSS)对患者心理压力进行量化评估,同时收集一般人口学信息、疾病及治疗情况,以及心理和社会因素。结果 本次研究共收集并纳入研究206例手外伤患者资料,CPSS评分为31.58±8.12分。在一般资料中,性别以及家庭人均收入对患者的CPSS评分存在显著差异(P<0.05)。在疾病与治疗相关因素方面,不同损伤类型、合并神经损伤与治疗方式对患者的CPSS评分比较也显示出显著差异(均P<0.05)。在心理及社会因素方面,患者的心理弹性及护理满意度不同亦表现出CPSS评分的显著性差异。单因素相关分析结果显示,患者的心理压力评分与性别、家庭人均收入、损伤类型、合并神经损伤、治疗方式、心理弹性及护理满意度存在相关性(P<0.05)。在进行多元线性回归分析时,治疗方式被排除,结果表明,患者的性别、家庭人均收入、损伤类型、合并神经损伤、心理弹性及护理满意度是影响心理压力评分的独立危险因素。结论 手外伤患者心理压力普遍较大,临床中除关注患者手部治疗及康复锻炼外,还应重视患者的心理状况,性别、家庭人均收入、损伤类型、合并神经损伤、患者心理弹性及护理满意度均是影响患者心理压力水平的独立危险因素,临床中应根据实际情况,提高对上述因素的管理效率,及时调整治疗及护理措施,改善患者心理压力。Objective To explore the level of psychological stress and its influencing factors in patients with hand trauma.Methods A crosssectional study was conducted.The clinical data of 206 patients with hand trauma admitted to our hospital from February 2021 to January 2024 were collected by convenient sampling method.The psychological pressure[Chinese version of the Perceived Stress Scale(CPSS)],general data,disease and treatment,psychological and social factors of patients were counted by scale.Results A total of 206 patients with hand trauma were collected and included in this study.The CPSS score was 31.58±8.12.There were significant differences in CPSS scores between patients with different gender and family per capita income in general data(P<0.05).There were significant differences in CPSS scores between patients with different types of injury,combined nerve injury and different treatment methods(all P<0.05).Among the psychological and social factors,there were statistically significant differences in CPSS scores among patients with different resilience and nursing satisfaction.The results of single factor correlation analysis showed that the score of psychological stress was correlated with gender,family per capita income,injury type,combined nerve injury,treatment method,resilience and nursing satisfaction(P<0.05).After the above factors were included in the multiple linear regression analysis,the treatment methods were excluded.The patient's gender,family per capita income,type of injury,combined nerve injury,psychological resilience and nursing satisfaction were independent risk factors for psychological stress scores.Conclusion The psychological pressure of patients with hand trauma is generally high.In addition to paying attention to the hand treatment and rehabilitation exercise of patients,we should also pay attention to the psychological status of patients.Gender,family per capita income,type of injury,combined nerve injury,patients'resilience and nursing satisfaction are all independent risk factors af
