PCI术后患者社会支持对复发恐惧的影响:家庭韧性与创伤后成长的链式中介效应

摘　　要：目的 探讨社会支持对PCI术后患者复发恐惧的影响,以及家庭韧性和创伤后成长在其中的中介机制。方法 于2023年8月~2024年3月,使用方便抽样的方法。采用一般情况调查表、恐惧疾病进展简化量表、社会支持评定量表、家庭韧性评估量表、创伤后成长评定量表对210名PCI术后患者进行调查。结果 相关分析显示,社会支持与PCI术后患者的家庭韧性、创伤后成长、均显著正相关(r=0.573,0.542;P<0.01);社会支持与PCI术后患者的复发恐惧显著负相关(r=-0.627,P<0.01);PCI术后患者家庭韧性与其创伤后成长显著正相关(r=0.437,P<0.01);与复发恐惧显著负相关(r=-0.566,P<0.01);PCI术后患者的创伤后成长与复发恐惧显著负相关(r=-0.642,P<0.01)。社会支持可以直接预测PCI术后患者的复发恐惧,也可以通过PCI术后患者的家庭韧性和创伤后成长的中介作用,以及这两者所形成的链式中介效应所产生的间接效应来发挥作用。结论 PCI术后患者家庭韧性和创伤后成长是社会支持对PCI术后患者复发恐惧影响的中介机制。Objective To explore the effect of social support on fear of recurrence in patients after PCI,and the mediation mechanism of family resilience and post-traumatic growth.Methods From August 2023 to March 2024,the convenient sampling method was used.A total of 200 patients after PCI were investigated by general situation questionnaire,fear of progression questionnaire short form,social support rating scale,family resilience assessment scale,post-traumatic growth inventory.Results Correlation analysis showed that social support was significantly positively correlated with family resilience and post-traumatic growth in patients after PCI(r=0.573,r=0.542,P<0.01);social support was significantly negatively correlated with fear of recurrence in patients after PCI(r=-0.627,P<0.01);family resilience was significantly positively correlated with post-traumatic growth in patients after PCI(r=0.437,P<0.01);and was significantly negatively correlated with fear of recurrence(r=-0.566,P<0.01);posttraumatic growth was significantly negatively correlated with fear of recurrence in patients after PCI(r=-0.642,P<0.01).Mediation effect analysis showed that social support could not directly predict fear of recurrence in patients after PCI,but played an indirect role through the mediation effect of family resilience and post-traumatic growth in patients after PCI,as well as the indirect effect of the chain mediation effect formed by the two.Conclusion Family resilience and post-traumatic growth in patients after PCI are the mediation mechanism of social support on fear of recurrence in patients after PCI.
