摘 要:目的了解2021—2023年包头市白云鄂博矿区儿童、孕妇碘营养状况,并探讨其影响因素。方法采用分层随机抽样方法,调查白云鄂博矿区8~10岁儿童600名、孕妇53名,采集其尿液样本及食用盐盐样进行测定。结果2021—2023年共检测白云鄂博矿区8~10岁儿童及孕妇家中盐样653份,碘盐覆盖率为100%,合格碘盐食用率为99.8%。8~10岁儿童尿样600份,尿碘中位数为182.30μg/L,处于碘充足状态,不同年份、年龄段之间尿碘分布水平差异有统计学意义(P<0.05)。检测孕妇尿样53份,不同年份孕妇尿碘中位数分别为241.25、156.21和145.70μg/L,不同年份之间孕妇尿碘分布水平差异有统计学意义(P<0.05)。结论2021—2023年包头市白云鄂博矿区儿童尿碘处于适宜水平,而孕妇尿碘中位数各年分别处于碘超适宜、碘适宜及碘缺乏范围,因此需要精细化儿童及孕妇健康教育,定期监测儿童、孕妇碘营养状况,避免碘摄入不足或过量带来的相关健康风险负担。Objective To assess the iodine nutritional status of children and pregnant women in the Baiyun Obo mining area of Baotou City from 2021 to 2023,and to explore the influencing factors.Methods A stratified random sampling method was used to investigate 600 children aged 8-10 years and 53 pregnant women in the Baiyun Obo mining area.Urine samples and edible salt samples were collected and analyzed.Results A total of 635 salt samples from the households of children aged 8-10 years and pregnant women in the Baiyun Obo mining area were tested from 2021 to 2023.The iodized salt coverage rate was 100%,and the consumption rate of qualified iodized salt was 99.8%.A total of 600 urine samples from children aged 8-10 years were tested,with a median urinary iodine concentration of 182.30μg/L,indicating adequate iodine levels.There were statistically significant differences in urinary iodine distribution levels between different years and age groups(P<0.05).Urine samples from 53 pregnant women were tested,and the median urinary iodine concentrations in different years were 241.25,156.21 and 145.70μg/L,respectively.There were statistically significant differences in urinary iodine distribution levels among pregnant women across different years(P<0.05).Conclusion From 2021 to 2023,the urinary iodine levels of children in the Baiyun Obo mining area of Baotou City were within an adequate range.However,the median urinary iodine concentrations in pregnant women over the three years were within the ranges of iodine excess,adequacy and deficiency,respectively.Therefore,there is a need for targeted health education for children and pregnant women,along with regular monitoring of their iodine nutritional status,to prevent the health risks associated with both insufficient and excessive iodine intake.
