摘 要:目的:探讨钆特酸葡胺在脑转移瘤患者磁共振成像(MRI)增强扫描中的应用效果及安全性。方法:回顾性分析2022年1月—2024年7月成都市温江区人民医院收治的60例脑转移瘤患者资料,根据对比剂类型的不同将其分为对照组及研究组,各30例。对照组采用离子型线性钆对比剂钆贝葡胺,研究组采用离子型大环状钆对比剂钆特酸葡胺。比较不同对比剂MRI增强扫描在脑转移瘤MRI检查中的诊断效果及安全性。结果:研究组的及时增强、延迟增强时病灶实性部分相对信号强度比均高于对照组,差异均有统计学意义(P<0.05)。研究组的小脑齿状核或苍白球信号强度比增加比例低于对照组,差异均有统计学意义(P<0.05)。研究组的不良事件发生率为3.33%,低于对照组的20.00%;研究组的不良反应发生率为3.33%,低于对照组的26.67%,差异均有统计学意义(P<0.05)。结论:大环状钆对比剂钆特酸葡胺对脑转移瘤的检出能力显著,且安全性更佳。Objective:To evaluate the efficacy and safety of gadoteric acid meglumine salt in enhanced magnetic resonance imaging(MRI)scanning in patients with brain metastases.Methods:Data of 60 patients with brain metastases admitted to Chengdu Wenjiang District People’s Hospital from January 2022 to July 2024 were retrospectively analyzed.According to different types of contrast agents,they were divided into a control group and a study group,with 30 cases in each group.The control group was treated with gadobenate dimeglumine,an ionic linear gadolinium contrast agent,and the study group was treated with gadoteric acid meglumine salt,an ionic large ring gadolinium contrast agent.The diagnostic efficacy and safety of MRI enhanced scanning with different contrast agents in MRI of brain metastases were compared.Results:The relative signal intensity ratio of the real part of the lesion at the timely enhancement and delayed enhancement in the study group were higher than those in the control group,and the differences were statistically significant(P<0.05).The increase ratio of signal intensity in the dentate nucleus or pallidum of cerebellum in the study group was lower than that in the control group,and the differences were statistically significant(P<0.05).The incidence of adverse events in the study group was 3.33%,which was lower than 20.00%in the control group;the incidence of adverse reactions in the research group was 3.33%,which was lower than the control group’s 26.67%,and the differences were statistically significant(P<0.05).Conclusion:Gadoteric acid meglumine salt,a large ring gadolinium contrast agent,has a significant ability to detect brain metastases with better safety.
