摘 要:目的观察改良真空辅助闭合(VAC)技术治疗糖尿病足溃疡(DFU)的临床效果。方法选取2023年9月至2024年9月湖南省第三人民医院和岳阳市中医医院收治入院的40例DFU患者作为研究对象,随机将其分为对照组和治疗组,每组20例。对照组采用常规治疗+传统清创治疗,治疗组采用常规治疗+改良VAC技术治疗。比较两组的治疗效果。结果治疗后,两组的足部溃疡面积均小于治疗前,且治疗组小于对照组(P<0.05)。治疗组的创面愈合总有效率明显高于对照组(P<0.05)。治疗后,两组的足部皮温、踝肱指数(ABI)明显高于治疗前,且治疗组的ABI高于对照组(P<0.05),但两组的足部皮温比较,差异无统计学意义(P>0.05);治疗后,两组的视觉模拟评分(VAS)评分均低于治疗前,且治疗组低于对照组(P<0.05)。治疗后,两组的血管内皮生长因子(VEGF)水平明显高于治疗前,基质金属蛋白酶-2(MMP-2)、基质金属蛋白酶-9(MMP-9)水平明显低于治疗前(P<0.05);治疗后,治疗组的VEGF水平明显高于对照组,MMP-2、MMP-9水平明显低于对照组(P<0.05)。结论改良VAC技术可以有效地缩小DFU面积,提升创面愈合率,改善创面血供,缓解疼痛。Objective To observe the clinical effect of modified vacuum-assisted closure(VAC)technology in the treatment of diabetic foot ulcer(DFU).Methods From September 2023 to September 2024,40 patients with DFU admitted in the Third People's Hospital of Hunan Province and Yueyang Traditional Chinese Medicine Hospital were selected as the study objects,the patinets were randomly divided into control group and treatment group,with 20 cases in each group.The control group was treated with conventional therapy+traditional debridement,and the treatment group was treated with conventional therapy+modified VAC technology.The therapeutic effects of the two groups were compared.Results After treatment,the foot ulcer area of the two groups was smaller than that before treatment,and that of the treatment group was smaller than the control group(P<0.05).The total effective rate of wound healing in the treatment group was significantly higher than that in the control group(P<0.05).After treatment,the foot skin temperature and ankle brachial index(ABI)of the two groups were significantly higher than those before treatment,and the ABI of the treatment group was higher than that of the control group(P<0.05),but there was no significant difference in foot skin temperature between the two groups(P>0.05);after treatment,the Visual Analogue Scale(VAS)scores of the two groups was lower than that before treatment,and that in the treatment group were lower than the control group(P<0.05).After treatment,the levels of vascular endothelial growth factor(VEGF)in the two groups was significantly higher than that before treatment,and the levels of matrix metalloproteinase-2(MMP-2)and matrix metalloproteinase-9(MMP-9)were significantly lower than those before treatment(P<0.05);after treatment,the VEGF level in the treatment group was significantly higher than that in the control group,and the MMP-2 and MMP-9 levels were significantly lower than those in the control group(P<0.05).Conclusion Modified VAC technology can effectively reduce the area of D
