摘 要:目的探讨重组人生长激素(rhGH)治疗对特发性矮小(ISS)患儿各项眼部生物学参数的影响。方法选取2020年6月至2022年8月于安徽医科大学第一附属医院儿科内分泌病区就诊并接受rhGH治疗的11例ISS患儿作为ISS治疗组,保守治疗的6例患儿作为ISS非治疗组,同时选取59名健康儿童作为正常对照组。ISS治疗组按照内分泌医师指导进行rhGH治疗,ISS非治疗组采用改善饮食结构及生活习惯、鼓励户外运动等保守治疗。比较三组研究对象的眼部生物学参数。结果随访开始时、随访6个月,正常对照组的眼轴长度大于ISS治疗组及ISS非治疗组(P<0.05);随访12个月,ISS治疗组的眼轴长度明显大于正常对照组和ISS非治疗组(P<0.05)。随访6、12个月,ISS治疗组的眼压大于ISS非治疗组,ISS非治疗组的眼压大于正常对照组,差异具有统计学意义(P<0.05)。随访6、12个月,ISS治疗组的角膜曲率大于ISS非治疗组及正常对照组,差异具有统计学意义(P<0.05)。结论ISS患儿未治疗前存在眼轴偏短的问题,而rhGH治疗有促进眼轴增长、升高眼压、增大角膜曲率的作用。Objective To investigate the effect of recombinant human growth hormone(rhGH)on ocular biological parameters in children with idiopathic short stature(ISS).Methods From June 2020 to August 2022,11 children with ISS who were treated with rhGH in the pediatric endocrine ward of the First Affiliated Hospital of Anhui Medical University were selected as ISS treatment group,6 children with conservative treatment were selected as ISS non-treatment group,and 59 healthy children were selected as normal control group.The ISS treatment group was treated with rhGH according to the guidance of endocrinologists,and the ISS non-treatment group was treated with conservative treatment such as improving dietary structure and living habits and encouraging outdoor sports.The ocular biological parameters of the three groups of research objects were compared.Results At the beginning of follow-up and 6 months of follow-up,the axial length of the normal control group was longer than that of the ISS treatment group and the ISS non-treatment group(P<0.05);at 12 months of follow-up,the axial length of the ISS treatment group was significantly longer than that of the normal control group and the ISS non-treatment group(P<0.05).At 6 and 12 months of follow-up,the intraocular pressure in the ISS treatment group was higher than that in the ISS non-treatment group,and the intraocular pressure in the ISS non-treatment group was higher than that in the normal control group,the differences were statistically significant(P<0.05).At 6 and 12 months of follow-up,the corneal curvature of the ISS treatment group was greater than that of the ISS non-treatment group and the normal control group,and the difference was statistically significant(P<0.05).Conclusion Children with ISS have a problem of short axial length before treatment,while rhGH treatment can promote axial growth,elevate intraocular pressure,and increase corneal curvature.
