老年2型糖尿病伴牙周炎患者口腔菌群分布及龈沟液MIF、HIF-1α、β-catenin水平分析

摘  要:目的分析老年2型糖尿病伴牙周炎患者口腔菌群分布及龈沟液巨噬细胞移动抑制因子(MIF)、低氧诱导因子-1α(HIF-1α)、β-catenin水平。方法选取2022年5月至2023年6月收治的64例老年2型糖尿病伴牙周炎患者作为观察组,另选取同期收治的64例单纯老年牙周炎患者作为对照组。比较两组的牙周健康状况、口腔菌群分布情况及龈沟液MIF、HIF-1α、β-catenin水平。结果观察组的牙周袋深度(PD)、牙周附着丧失(AL)显著大于对照组,菌斑指数(PLI)、龈沟出血指数(SBI)评分显著高于对照组,差异具有统计学意义(P<0.05)。观察组的二氧化碳嗜纤维菌、齿垢密螺旋体、口腔链球菌、具核梭杆菌、中间普氏菌及牙龈卟啉单胞菌检出率显著高于对照组,差异具有统计学意义(P<0.05)。观察组的龈沟液MIF、HIF-1α及β-catenin水平显著高于对照组,差异具有统计学意义(P<0.05)。结论老年2型糖尿病伴牙周炎患者的牙周健康状况较差,口腔内病原菌群分布更为广泛,且龈沟液中MIF、HIF-1α和β-catenin水平显著升高。在临床诊疗中,需重点关注上述指标。Objective To analyze the distribution of oral flora and the levels of macrophage migration inhibition factor(MIF),hypoxia-inducible factor-1α(HIF-1α)andβ-catenin in gingival crevicular fluid of elderly patients with type 2 diabetes mellitus and periodontitis.Methods Sixty-four elderly patients with type 2 diabetes mellitus and periodontitis admitted from May 2022 to June 2023 were selected as observation group,and 64 elderly patients with simple periodontitis admitted during the same period were selected as control group.The periodontal health status,distribution of oral flora and levels of MIF,HIF-1αandβ-catenin in gingival crevicular fluid were compared between the two groups.Results The probing depth(PD)and attachment loss(AL)in the observation group were significantly greater than those in the control group,and the plaque index(PLI)and sulcus bleeding index(SBI)scores were significantly higher than those in the control group,the differences were statistically significant(P<0.05).The detection rates of Capnocytophaga,Treponema denticola,Streptococcus oralis,Fusobacterium nucleatum,Prevotella intermedia and Porphyromonas gingivalis in the observation group were significantly higher than those in the control group,and the differences were statistically significant(P<0.05).The levels of MIF,HIF-1αandβ-catenin in gingival crevicular fluid of the observation group were significantly higher than those of the control group,and the differences were statistically significant(P<0.05).Conclusion The periodontal health status of elderly patients with type 2 diabetes mellitus and periodontitis is poor,the distribution of oral pathogenic bacteria is more extensive,and the levels of MIF,HIF-1αandβ-catenin in gingival crevicular fluid are significantly increased.In clinical diagnosis and treatment,we need to focus on the above indicators.
