天麻钩藤饮联合帕罗西汀治疗肝阳上亢型PPPD患者的有效性及安全性

摘　　要：目的分析天麻钩藤饮联合帕罗西汀治疗肝阳上亢型持续性姿势知觉性头晕(PPPD)的临床效果。方法选取我院2023年2月至8月收治的60例肝阳上亢型PPPD患者作为研究对象,以治疗方案差异将其分为对照组(30例,帕罗西汀)和研究组(30例,天麻钩藤饮联合帕罗西汀)。比较两组的头晕症状改善情况、血流动力学及不良反应发生情况。结果治疗后,两组的头晕发作频率、眩晕残障程度评定量表(DHI)评分显著降低,头晕发作持续时间显著缩短,且研究组优于对照组,差异具有统计学意义(P<0.05)。治疗后,两组的左椎动脉、右椎动脉、基底动脉血流速度显著增大,阻力指数、红细胞压积(HCT)及血浆黏度(PV)显著降低,且研究组优于对照组,差异具有统计学意义(P<0.05)。两组的不良反应总发生率比较,差异无统计学意义(P>0.05)。结论天麻钩藤饮联合帕罗西汀治疗肝阳上亢型PPPD有助于改善患者头晕症状,缩短头晕症状持续时间,改善血流动力学,且不良反应少,值得临床推广及应用。Objective To analyze the clinical effect of Tianma Gouteng decoction combined with paroxetine in the treatment of persistent postural perceptual dizziness(PPPD)with liver-yang hyperactivity type.Methods From February to August 2023,60 patients with PPPD of liver-yang hyperactivity type admitted in our hospital were selected as the research objects.According to the difference of treatment plan,the patients were divided into control group(30 cases,paroxetine)and study group(30 cases,Tianma Gouteng decoction combined with paroxetine).The improvement of dizziness symptoms,hemodynamics and adverse reactions were compared between the two groups.Results After treatment,the frequency of dizziness attack and Dizziness Handicap Inventory(DHI)score of the two groups reduced significantly,the duration of dizziness attack shortened significantly,and those in the study group were better than the control group,the differences were statistically significant(P<0.05).After treatment,the blood flow velocity of left vertebral artery,right vertebral artery and basilar artery in the two groups increased significantly,the resistance index,hematocrit(HCT)and plasma viscosity(PV)decreased significantly,and those in the study group were better than the control group,the differences were statistically significant(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion Tianma Gouteng decoction combined with paroxetine in the treatment of PPPD of liver-yang hyperactivity type is helpful to improve the dizziness symptoms of patients,shorten the duration of dizziness symptoms,and improve hemodynamics with fewer adverse reactions,which is worthy of clinical promotion and application.
