摘 要:目的探讨精益管理模式在缩短急诊生化标本周转时间(TAT)中的应用效果。方法将通过实验室信息管理系统收集的1001份急诊生化标本作为研究样本,其中2023年10月1日至10月31日为优化前(508份);2023年11月1日至11月30日实施精益管理模式为优化后(493份)。比较精益管理优化前、后的急诊生化TAT1(标本签收至核收时间)、TAT2(核收至出报告时间)、TAT3(报告至审核时间)和总TAT(标本签收至标本审核时间)及TAT第90百分位数不合格率情况。结果优化后,TAT1、TAT2和总TAT均短于优化前,差异具有统计学意义(P<0.01)。总TAT第90百分位数不合格率从优化前的18.31%下降到优化后的7.71%(P<0.01)。不合格标本TAT1第90百分位数从优化前的616.10 min缩短到193.80 min;TAT2第90百分位数从优化前的124.20 min缩短到104.40 min;总TAT第90百分位数从优化前的722.50 min缩短到300.00 min。夜间非急诊项目占比从优化前的3.15%下降到优化后的0.61%。优化后,高峰时间段TAT1、TAT2和总TAT显著短于优化前(P<0.05)。结论临床实验室可通过精益化管理模式优化检验流程,缩短急诊生化标本TAT。Objective To explore the application effect of lean management mode in shortening the turnaround time(TAT)of emergency biochemical specimens.Methods A total of 1001 emergency biochemical specimens collected through the laboratory information management system were used as research samples,of which from October 1 to October 31,2023 was before optimization(508 copies);the lean management model was implemented from November 1 to November 30,2023(after optimization,493 copies).The TAT1(time from specimen receipt to verification),TAT2(time from verification to report),TAT3(time from report to audit),total TAT(time from specimen receipt to specimen audit)and TAT 90th percentile failure rate of emergency biochemistry before and after lean management optimization were compared.Results After optimization,TAT1,TAT2 and total TAT were shorter than those before optimization,and the differences were statistically significant(P<0.01).The 90th percentile failure rate of total TAT decreased from 18.31%before optimization to 7.71%after optimization(P<0.01).The 90th percentile of TAT1 unqualified apecimens was shortened from 616.10 min before optimization to 193.80 min;and the 90th percentile of TAT2 was shortened from 124.20 min before optimization to 104.40 min;the 90th percentile of total TAT was shortened from 722.50 min before optimization to 300.00 min.The proportion of nighttime non-emergency projects decreased from 3.15%before optimization to 0.61%after optimization.After optimization,TAT1,TAT2 and total TAT in the peak period were significantly shorter than those before optimization(P<0.05).Conclusion Clinical laboratories can optimize the inspection process through lean management mode and shorten the TAT of emergency biochemical specimens.
