微激光开孔辅助卵胞浆内单精子显微注射技术的临床研究

摘  要:目的:观察低能量激光对卵子透明带进行开孔辅助卵胞浆内单精子显微注射技术(ICSI)的临床研究。方法:选取2022年1月1日-2024年4月31日在九江市妇幼保健院生殖中心施行第二代试管婴儿技术治疗的患者202例,根据治疗方法不同分为常规ICSI组(以下简称A组,n=81,注射卵子数652个)、开孔深度<50%组(以下简称B组,n=60,注射卵子数526个)和开孔深度≥50%组(以下简称C组,n=61,注射卵子数535个)。A组采用常规ICSI治疗,B组采用微激光开孔深度<50%辅助ICSI治疗,C组采用微激光开孔深度≥50%辅助ICSI治疗。治疗后比较三组受精率、(2PN)正常受精率(2PN指卵子正常受精后出现雌雄双原核)、优质胚胎率、临床妊娠率及囊胚形成率。结果:A组受精率、(2PN)正常受精率、优质胚胎率低于B组和C组,B组受精率、优质胚胎率低于C组,差异均有统计学意义(P<0.05),B组和C组(2PN)正常受精率比较,差异无统计学意义(P>0.05)。A组临床妊娠率、囊胚形成率低于B组和C组,B组临床妊娠率、囊胚形成率低于C组,差异均有统计学意义(P<0.05)。结论:微激光开孔辅助ICSI技术运用于试管婴儿中,可有效提高受精率、优质胚胎率、临床妊娠率和囊胚形成率。Objective:To observe the clinical study of intracytoplasmic sperm injection(ICSI)assisted by microlaser trepanning.Methods:A total of 202 patients treated with second-generation in vitro fertilization in reproductive center of Jiujiang Maternal and Child Health Hospital were enrolled from January 1,2022 to April 31,2024.According to different treatment methods,patients were divided into routine ICSI group(hereinafter referred to as Group A,n=81,number of injected ovum:652),trepanningdepth<50%group(hereinafter referred to as Group B,n=60,number of injected ovum:526)and trepanningdepth≥50%group(hereinafter referred to as Group C,n=61,number of injected ovum:535).Group A was treated with conventional ICSI,group B was treated with micro-laser drilling depth<50%and group C was treated with micro-laser drilling depth≥50%.After treatment,fertilization rate,(2PN)normal fertilization rate(2PN is after normal fertilization of the egg,the male and female prokaryotes appear),high-quality embryo rate,clinical pregnancy rateand blastocyst formation rate of three groups were compared.Results:The fertilization rate,(2PN)normal fertilization rate and high-quality embryo rate in group A were lower than those in group B and group C.The fertilization rate and high-quality embryo rate in groupB were lower than those in group C(P<0.05),but there was no significant difference in(2PN)normal fertilization rate between the two groups(P>0.05).The clinical pregnancy rate and blastocyst formation rate in group A were lower than those in group B and group C,and the clinical pregnancy rate and blastocyst formation rate in group B were lower than those in group C(P<0.05).Conclusion:The application of ICSI assisted by microlaser trepanningin IVF can effectively improve fertilization rate,high-quality embryo rate,clinical pregnancy rate and blastocyst formation rate.
