肥胖儿童糖脂代谢状况及其与性早熟的相关性

摘  要:目的:探讨肥胖儿童糖脂代谢状况,并分析糖脂代谢状况与儿童发生性早熟的相关性。方法:选取2018年3月-2021年3月符合纳入标准的肥胖儿童65例(肥胖组)与体质量正常儿童65例(对照组),检测两组糖脂代谢相关指标,参照谭纳(Tanner)分期标准对两组儿童进行性早熟评估,分析糖脂代谢对肥胖儿童发生性早熟的影响。结果:肥胖组血糖代谢各项指标均高于对照组(P<0.05);肥胖组甘油三酯(TG)、总胆固醇(TC)、低密度脂蛋白(LDL)、尿酸(UA)高于对照组,高密度脂蛋白(HDL)低于对照组(P<0.05);肥胖组男童、女童Tanner评分均高于对照组,性早熟发生率高于对照组(P<0.05);Spearman相关系数检验结果显示,肥胖组男童睾丸、阴毛Tanner评分和女童乳房、阴毛Tanner评分均与空腹血糖(FPG)、餐后2 h血糖(2 h PG)、糖化血红蛋白(HbAlc)、空腹胰岛素(FINS)、胰岛素抵抗指数(HOMA-IR)及TG,TC,LDL,UA呈正相关(P<0.05),与HDL呈负相关(P<0.05);Logistic回归分析结果显示,FPG,GHb,FINS,HOMA-IR,TG,TC,UA水平升高是肥胖儿童发生性早熟的危险因素(OR>1,P<0.05)。结论:肥胖与性早熟存在关联,且肥胖儿童糖脂代谢紊乱与性早熟密切相关。Objective:To investigate the status of glucose and lipid metabolism in obese children and analyze the effect of glucose and lipid metabolism on precocious puberty in children.Methods:All 65 obese children(obesity group)and 65 normal weight children(control group)who met the inclusion criteria from March 2018 to March 2021 were selected.The indexes related to glucose and lipid metabolism were detected in the two groups.According to Tanner's staging standard,the precocious puberty of two groups of children was evaluated,and the influence of glucose and lipid metabolism on precocious puberty of obese children was analyzed.Results:The indexes of blood glucose metabolism in obesity group were higher than those in control group(P<0.05).The triglyceride(TG),total cholesterol(TC),low density lipoprotein(LDL)and uric acid(UA)in obesity group were higher than those in control group,while high density lipoprotein(HDL)was lower than that in control group(P<0.05).The Tanner scores of boys and girls in obesity group were higher than those in control group,and the incidence of precocious puberty was higher than that in control group(P<0.05).By Spearman correlation coefficient test,the results showed that the Tanner scores of testis and pubic hair in boys and breast and pubic hair in girls were positively correlated with fasting blood glucose(FPG),2 h postprandial blood glucose(2 h PG),glycosylated hemoglobin(HbAlc),fasting insulin(FINS),insulin resistance index(HOMA-IR),TG,TC,LDL and UA(P<0.05),and negatively correlated with HDL(P<0.05).By Logistic regression analysis,the results showed that the elevated levels of FPG,GHB,fins,HOMA-IR,TG,TC and UA were the risk factors of precocious puberty in obese children(OR>1,P<0.05).Conclusion:Obesity is associated with precocious puberty,and the disorder of glucose and lipid metabolism is an important factor to promote precocious puberty in obese children.
