摘　　要：目的:探讨胞磷胆碱钠片联合高压氧(HBO)治疗重度创伤性脑损伤(TBI)认知功能障碍的临床效果。方法:选取2018年1月-2024年3月泉州市第一医院收治的重度TBI患者94例,按照随机数字表法分为观察组与对照组,每组47例。对照组给予HBO治疗,观察组给予胞磷胆碱钠片联合HBO治疗,比较两组临床疗效、认知功能、脑血流动力学及神经因子水平。结果:观察组总有效率高于对照组(P<0.05)。两组治疗后简易精神状态检查量表(MMSE)评分、蒙特利尔认知评估量表(MoCA)评分、脑血流量(CBF)、脑血容量(CBV)、脑源性神经营养因子(BDNF)高于治疗前(P<0.05),且观察组高于对照组(P<0.05)。两组治疗后神经元特异性烯醇化酶(NSE)低于治疗前(P<0.05),且观察组低于对照组(P<0.05)。结论:胞磷胆碱钠片联合HBO治疗重度TBI认知功能障碍,能够提高临床疗效,促进患者认知功能恢复,改善脑血流动力学及神经因子水平。Objective:To explore the clinical efficacy of citicoline sodium tablets and hyperbaric oxygen(HBO)therapy for cognitive impairment in severe traumatic brain injury(TBI).Methods:A total of 94 patients with severe TBI from January 2018 to March 2024 who were admitted in Quanzhou First Hospital were randomly divided into the observation group(n=47,Citicoline Sodium Tablets and HBO)and the control group(n=47,HBO).The clinical efficacy,cognitive function,cerebral hemodynamics,and nerve factors in both groups were compared.Results:The total effective rate of the observation group was higher than that of the control group(P<0.05).After treatment,the scores of mini mental state examination(MMSE),montreal cognitive assessment scale(MoCA),cerebral blood flow(CBF),cerebral blood volume(CBV),and brain-derived neurotrophic factor(BDNF)in both groups were increased(P<0.05),and the observation group was higher than the control group(P<0.05).After treatment,the levels of neuron specific enolase(NSE)in both groups were decreased(P<0.05),and the observation group was lower than the control group,(P<0.05).Conclusion:The combination of citicoline sodium tablets and HBO in the treatment of severe TBI can improve clinical efficacy,promote cognitive function recovery,and improve cerebral hemodynamics and cytokine levels.
