基于甘苦合化理论运用清营汤辨治放射性皮炎营分证

摘  要:放射性皮炎引起的局部或全身急性损伤以及感染性并发症具有病程长、渐进加重、易复发、缠绵难愈的特点,西医学缺乏有效的应对手段。中医药在治疗放射性皮炎、提高患者生活质量等方面有一定优势。中医认为放射性皮炎属于“烧伤”范畴,其营分证阶段是治疗的重要阶段,治疗要点在于清热与养阴并举,邪正兼顾,故基于甘苦合化理论运用苦味药、甘味药以清泄热邪、合化阴气。甘苦合化法以清营汤为代表方剂,透热转气、祛除热毒,甘苦合化、滋阴生津,临床上并根据不同情况加减化裁,结合西医基础治疗,能够有效缓解放射性皮炎营分病症状,防止病情深入,截断、扭转病势。Radiodermatitis is characterized by acute local or systemic damage and infectious complications,with a long course,progressive worsening,tendency to recur,and difficulty in recovery.Modern medicine lacks effective treatments.Traditional Chinese Medicine(TCM)offers advantages in treating radiodermatitis and improving patient quality of life.TCM regards radiodermatitis as a type of burn,with the Nutrient aspect syndrome being a key stage in treatment.This paper believes that the treatment focus should be on both clearing heat and nourishing yin,addressing both pathogenic factor and healthy qi.Therefore,based on the theory of sweet-bitter combination,this approach uses bitter herbs combined with sweet herbs to clear and drain heat pathogen and harmonize yin qi.Utilizing the sweet-bitter combination method,Qingying Decoction is used as a representative formula to clear heat,transform qi,expel heat toxins,and nourish yin and generate fluids.Clinically,the formula can be modified according to different conditions,combined with conventional Western treatment,to effectively alleviate symptoms caused by Nutrient aspect syndrome,prevent disease progression,and arrest and reverse the disease process.
