益胃解毒汤治疗气虚血瘀型慢性萎缩性胃炎临床疗效观察

摘　　要：目的:探讨益胃解毒汤对气虚血瘀型慢性萎缩性胃炎的临床治疗效果。方法:选择2022年7月—2023年10月如皋市中医院收治的78例气虚血瘀型慢性萎缩性胃炎患者,依照随机数字表法将其分为常规组及研究组,每组39例。常规组采用常规四联治疗,研究组在常规组的基础上给予益胃解毒汤治疗。观察两组患者的中医证候积分、血清学指标、恢复情况、临床疗效、不良反应发生情况。结果:治疗后,研究组患者中医证候积分低于常规组(P<0.01);研究组胃蛋白酶原Ⅰ(PG-Ⅰ)、PG-Ⅰ/PG-Ⅱ均高于常规组(P<0.01);治疗后,两组IL-6、TNFR1水平均降低(P<0.05),且研究组低于常规组(P<0.01);两组G-17水平均升高(P<0.05),且研究组更高(P<0.01);研究组患者体倦乏力、脘腹胀满、胃脘疼痛症状消失时间均低于常规组(P<0.01);研究组患者治疗总有效率高于常规组(P<0.01);两组不良反应总发生率差异无统计学意义(P>0.05)。结论:益胃解毒汤能够改善患者血清学指标,降低中医证候积分,缩短患者恢复时间及临床疗效,安全性较高。Objective:To explore the clinical therapeutic effect of Yiwei Jiedu Decoction on Qi deficiency and blood stasis type chronic atrophic gastritis.Methods:A total of 78 patients with Qi deficiency and blood stasis type chronic atrophic gastritis,admitted to Rugao City Traditional Chinese Medicine Hospital from July 2022 to October 2023,were selected and randomly divided into two groups:the conventional treatment group and the study group,with 39 cases in each group.The conventional treatment group received routine quadruple therapy,while the study group was treated with Yiwei Jiedu Decoction in addition to conventional treatment.The two groups were compared in terms of Traditional Chinese Medicine(TCM)syndrome scores,serological indices,recovery time,clinical efficacy,and adverse reactions.Results:After treatment,the TCM syndrome score in the study group was significantly lower than that in the conventional group(P<0.01).The levels of gastric pepsinogen I(PG-I)and the ratio of PG-I/PG-II in the study group were significantly higher than those in the conventional group(P<0.01).After treatment,levels of TL-6 and TNFR1 decreased in both groups(P<0.05),with the study group showing a significantly greater reduction(P<0.01).Both groups showed an increase in G-17 levels(P<0.05),with the study group showing a higher increase(P<0.01).The time for symptoms such as fatigue,epigastric fullness,and stomach pain to resolve was shorter in the study group than in the conventional group(P<0.01).The total effective rate of treatment in the study group was higher than in the conventional group(P<0.05).The incidence of adverse reactions was similar between the two groups(P>0.05).Conclusion:Yiwei Jiedu Decoction can improve the serological indices,reduce TCM syndrome scores,shorten recovery time,and enhance clinical efficacy with high safety in the treatment of Qi deficiency and blood stasis type chronic atrophic gastritis.
