仁青芒觉胶囊联合四联疗法治疗胃溃疡的疗效及对胃黏膜保护、溃疡愈合的影响

摘　　要：目的观察仁青芒觉胶囊联合四联疗法治疗胃溃疡的疗效,并探讨其对患者胃黏膜保护、溃疡愈合的影响。方法选取2023年1月至2024年1月河南漯河市中心医院收治的112例胃溃疡患者作为研究对象,按随机数表法分为对照组和观察组各56例。对照组患者采用四联疗法治疗,观察组患者采用仁青芒觉胶囊联合四联疗法治疗,疗程两周。治疗后比较两组患者的疗效以及治疗前后的胃黏膜形态、溃疡愈合情况和炎症因子水平[白介素-6(IL-6)、白介素-10(IL-10)、转化生长因子-β(TGF-β)]、T淋巴细胞亚群(CD8^(+)、CD4^(+)、CD4^(+)/CD8^(+)),随访6个月,观察两组患者的胃溃疡复发情况。结果观察组患者的治疗总有效率为94.64%,明显高于对照组的82.14%,差异有统计学意义(P<0.05);观察组患者治疗1周后的腺体密度、黏膜厚度、慢性炎症浸润评分分别为(0.74±0.18)分、(0.69±0.15)分、(1.12±0.28)分,明显低于对照组的(0.98±0.26)分、(0.83±0.20)分、(1.46±0.32)分,治疗2周后腺体密度、黏膜厚度、慢性炎症浸润评分分别为(0.53±0.10)分、(0.42±0.09)分、(0.63±0.17)分,明显低于对照组的(0.82±0.13)分、(0.70±0.11)分、(1.12±0.26)分,差异均有统计学意义(P<0.05);观察组患者治疗1周后的溃疡底部厚苔面积、溃疡周围黏膜充血面积及再生上皮面积分别为(1.47±0.48)mm^(2)、(1.35±0.59)mm^(2)、(1.23±0.32)mm^(2),明显小于对照组的(2.36±0.67)mm^(2)、(2.12±0.74)mm^(2)、(1.98±0.77)mm^(2),治疗两周后溃疡底部厚苔面积、溃疡周围黏膜充血面积及再生上皮面积分别为(0.96±0.24)mm^(2)、(0.52±0.13)mm^(2)、(0.66±0.24)mm^(2),明显小于对照组的(1.75±0.47)mm^(2)、(1.36±0.39)mm^(2)、(1.02±0.35)mm^(2),差异均有统计学意义(P<0.05);观察组患者治疗1周后的血清IL-6、IL-10水平分别为(1165.32±76.39)pg/mL、(113.56±15.37)pg/mL,明显低于对照组的(1348.75±84.85)pg/mL、(129.78±17.20)pg/mL,TGF-β为(275.Objective To observe the efficacy of Renqing Mangjue Capsules combined with quadruple therapy in the treatment of gastric ulcer,and to explore its effects on gastric mucosal protection and ulcer healing in patients.Methods From January 2023 to January 2024,112 patients with gastric ulcer admitted to Luohe Central Hospital were selected as the study subjects and divided into a control group and an observation group,with 56 patients in each group,using the random number table method.Patients in the control group were treated with quadruple therapy,while those in the observation group were treated with Renqing Mangjue Capsules combined with quadruple therapy,for a course of 2 weeks.After treatment,the efficacy of the two groups of patients was compared,as well as the morphology of gastric mucosa,ulcer healing,and levels of inflammatory factors(interleukin-6[IL-6],interleukin-10[IL-10],transforming growth factor-β[TGF-β]),and T lymphocyte subsets(CD8^(+),CD4^(+),CD4^(+)/CD8^(+)).The patients in both groups were followed up for 6 months to observe the recurrence of gastric ulcer.Results The total effective rate of the observation group was 94.64%,which was significantly higher than 82.14%of the control group(P<0.05).The scores of glandular density,mucosal thickness,and chronic inflammatory infiltration in the observation group after 1 week of treatment were(0.74±0.18)points,(0.69±0.15)points,and(1.12±0.28)points,respectively,which were significantly lower than(0.98±0.26)points,(0.83±0.20)points,and(1.46±0.32)points in the control group;after 2 weeks of treatment,the scores were(0.53±0.10)points,(0.42±0.09)points,and(0.63±0.17)points,respectively,which were significantly lower than(0.82±0.13)points,(0.70±0.11)points,and(1.12±0.26)points in the control group;the differences were statistically significant(P<0.05).The area of thick fur on the bottom of the ulcer,the area of congestion in the surrounding mucosa,and the area of regenerated epithelium in the observation group after 1 week of treatment were(1.47
