西部地区农村家庭远程会诊服务的支付意愿及影响因素分析

摘　　要：目的从需求方角度探讨远程会诊服务发展的动力、支付意愿和影响因素,为相关政策可持续发展提供研究证据。方法利用条件价值法进行实地调查,通过Tobit回归、需求估计等方法分析支付意愿及其影响因素。结果在假设情境下,有62.63%(848/1354)和66.54%(901/1354)的农村家庭分别选择了使用远程会诊服务的形式咨询二级医院和三级医院医生,支付意愿的平均值分别为(87.00±109.25)元和(131.80±141.41)元,中位数分别为50元和90元。结论相当一部分农村家庭愿意选择远程医疗服务,尤其是三级医院医生提供的远程医疗服务。未来远程会诊服务的开展需要以居民的需求为中心,拓宽筹资渠道,丰富服务内容,建立可持续发展的运行机制。Objective To explore the driving force,willingness to pay,and influencing factors of the development of remote consultation services from the perspective of the demand side,thus providing research evidence for the sustainable development of relevant policies.Methods Field investigations were performed using the contingent valuation method.The willingness to pay and its influencing factors were explored through Tobit regression,demand estimation,and other methods.Results In the hypothetical scenario,62.63%(848/1354)and 66.54%(901/1354)of rural families chose to use remote consultation services to consult doctors in secondary and tertiary hospitals,respectively.The average payment of willingness was 87.00±109.25 yuan and 131.80±141.41 yuan,with the median of 50 yuan and 90 yuan in secondary and tertiary hospitals,respectively.Conclusion Residents in western rural areas have a rather high willingness to choose the remote consultation services,especially the services from doctors in tertiary hospitals.Remote consultation services should be provided based on the health demand,take maximum advantage of the online health service to strengthen the health system in western rural areas,enrich the service content,establish financing and sustainable development mechanisms.
