胆总管结石行腹腔镜下胆总管探查术后胆总管压力变化的研究

摘　　要：目的研究胆总管结石病人行腹腔镜下胆总管探查术后胆总管的压力变化情况。方法2022年1月~2022年12月我院就诊的胆总管结石病人17例,所有病人均行腹腔镜下胆总管探查术,进行了胆总管压力测量,术后随访8周,第2、3、4、8周经留置T管测量胆总管压力。结果17例病人中,1例因残余胆总管结石表现为持续的高胆总管压力水平,其余16例病人术后胆总管压力(17.0 cm H_(2)O)和第1周的胆总管压力(15.5 cmH_(2)O)与初始胆总管压力(19.7 cmH_(2)O)比较,差异无统计学意义(P>0.05);第2周(13.5 cmH_(2)O)、第3周(12.2 cmH_(2)O)、第4周(11.1 cmH_(2)O)和第8周(10.0 cmH_(2)O)的胆总管压力与初始胆总管压力比较,差异均有统计学意义(P<0.05)。结论胆总管结石病人行腹腔镜下胆总管探查术后从第2周开始胆总管压力较初始压力有明显下降。Objective To study the change of common bile duct pressure after laparoscopic common bile duct exploration in patients with common bile duct stones.Methods Clinical data of 17 patients with common bile duct stones treated in our hospital from January 2022 to December 2022 were collected.All patients underwent laparoscopic common bile duct exploration and common bile duct pressure measurement.The patients were followed up for 8 weeks after operation,and the common bile duct pressure was measured by indwelling T tube at 2,3,4 and 8 weeks.Results Among the 17 patients,1 patient showed persistent high common bile duct pressure due to residual common bile duct stones.In the remaining 16 patients,there were no significant difference between the initial common bile duct pressure(19.7 cm H_(2)O)and the common bile duct pressure at postoperative(17.0 cm H_(2)O)and week 1(15.5 cm H_(2)O)(P>0.05).There were significant differences between the initial common bile duct pressure and the common bile duct pressure at week 2(13.5 cm H_(2)O),3(12.2 cm H_(2)O),4(11.1 cm H_(2)O)and 8(10.0 cm H_(2)O)(P<0.05).Conclusion Compared to initial common bile duct pressure,the pressure of common bile duct began to decrease significantly from 2 weeks after laparoscopic common bile duct exploration in patients with common bile duct stones.
